56例口腔游离组织移植术后患者经鼻留置口咽吸痰管的方法与应用

摘  要:目的 探讨口腔游离组织移植术后经鼻留置口咽吸痰管用于抽吸口咽积存分泌物的应用及效果,以便更好的进行术后护理观察,亦有利于移植的游离组织愈合。方法 对56例17腔肿瘤患者行根治性切除术后用皮瓣或肌皮瓣移植修复口腔内组织缺损,行经鼻留置口咽吸痰管,强化口腔护理,方便患者体位的管理、游离皮瓣的观察与监测。结果56例患者除l例胸大肌肌皮瓣出现部分坏死外全部成活,无口腔感染,未见吸入性肺炎的发生。结论 口腔游离组织移植术后病人由于术后制动加之咽喉部水肿,以及病人疼痛吞咽困难等原因,大量分泌物积聚在咽腔,对皮瓣血运观察及愈合不利,也降低病人的舒适度。因此定时用鼻咽吸痰管进行抽吸口腔内的分泌物,配合口腔护理,预防口腔感染等并发症,对促进皮瓣愈合创造有利条件。Objective To explore the application and effects oftransnasal indwelling oropharyngeal suction catheter to pump oropharyngeal accumulated secretion for patients after oral cavity free tissue transplantation. It is good for post-operation nursing and flap coalescence. Methods For 56 oral cavity tumor patients, perform radical resection and then use flap or musculocutaneous flap transplantation to repair oral cavity tissue defect. Perform transnasal indwelling oropharyngeal suction catheter and strengthen oral cavity nursing to facility the management of patients' body position and the observation and monitoring of free flap. Results Among 56 patients, except 1 patients with ectopectoralis musculocutaneous flap partial necrosis, the others were all alive without oral cavity infection or occurrence of aspiration pneumonia. Conclusions After oral cavity free tissue transplantation, because of patients' post-operation retaining, laryngopharynx edema, paining and dysphagia and others, a lot of secretion were accumulated in pharyngeal cavity that was bad for observation of flap blood flow and coalescence and reduced patients'comfort. Therefore, perform nasopharynx suction to pump the secretion in oral cavity and associated with oral cavity nursing, prevent oral cavity infection and other complications and create good condition for flap coalescence.
