中药灌肠对CKD3～4期患者血清中分子尿毒症毒素β_2-MG清除的临床观察

摘　　要：目的：通过在对照组上加用自拟方进行中药灌肠,探讨中药灌肠对CKD3~4期患者肾功能的改善情况以及其对血清中分子尿毒症毒素β2-MG的清除情况。方法：对符合要求的病人随机分为对照组和灌肠组,观察灌肠后12周的临床指标。结果：灌肠组12周后中医证候改善的总有效率明显优于对照组（P〈0.05）,并且对于血中SCr、BUN以及β2-微球蛋白的浓度的降低也明显优于对照组（P〈0.05）。结论：中药灌肠可有效改善患者肾功能情况,并进一步说明本自拟方中药灌肠不仅可以降低血中SCr、BUN等小分子尿毒症毒素的浓度,更可以降低血中β2-微球蛋白等中分子尿毒症毒素的浓度。可有效配合血液透析滤过等肾脏替代治疗。Objective ：To find out the improvement of renal function and the influence on the medium molecular substance （MMS） such as β2 -MG in blood for 3rd - 4th period patients of chronic kidney disease, we took the way as in the control group by the addition of traditional Chinese medicine enema. Methods ： We randomly divided the patients that met the requirements of the outpatients and inpatients in our hospital urology department into group and the control group, Gobserving the related clinical index 12 weeks later. Results：Results 12 weeks after the beginning of taking enema, the total effective rate of the improvement of TCM syndrome of the enema group is much higher than the control group （P 〈 0.05 ）. Furthermore, the decrease of the concentra- tion in blood of SCr,BUN and β2 - MG in enema group is much more than that in the control group（P 〈0. 05）. Conclusion：The Traditional Chinese Medicine enema could improve the renal function of the patients. Further more it could not only decrease theconcentration in blood of micromolecular uremic toxins such as SCr, BUN, but also reduce the concentration in blood of middlemolecular uremic toxins such as β2 - MG, and could effectively cooperate with renal replacement therapy such as hemodiafihration.
