四联疗法治疗头面部带状疱疹临床回顾性分析

摘　　要：目的:总结2012年1月—2013年8月我科诊治的64例头面部带状疱疹的临床特点及四联疗法的临床疗效。方法:分析头面部带状疱疹患者临床资料,运用四联疗法即中药、抗病毒药物、维生素与皮质激素联合应用治疗64例头面部带状疱疹患者,并重点分析治疗方案。结果:头面部带状疱疹64例患者中,依据皮损、并发症及个体差异确定病情严重程度,并排除有激素禁忌证后,予四联疗法治疗,痊愈38例(59.37%),显效17例(26.56%),有效8例(12.50%),死亡1例(1.56%)。总有效率达85.94%。64例患者中在治疗前CD3、CD4及CD4/CD8比值均有不同程度的下降,CD8则有不同程度的升高,在治疗后(除死亡1例外),CD3、CD4均较前有明显升高,CD8有所下降,但较前无显著性差异。结论:中西医结合四联疗法治疗头面部带状疱疹临床疗效满意,可有效抗病毒、减轻炎症,调节细胞免疫,阻止对神经的毒性及破坏作用,减少后遗神经痛的发生。Objective ： Clinical efficacy of clinical characteristics and quadruple therapy in 64 patients with craniofaeial herpes zoster in 2012 January to 2013 August was summarized in our hospital. Methods： Analysis of head and facial herpes zoster patients＇ clinical data was made by using the quadruple therapy, including Chinese traditional medicine, antiviral drugs, vitamins and eortieosteroid therapy in 64 patients with herpes zoster on head and face, and focused on the analysis of treatment. Results ： Head and facial herpes zoster in 64 patients, the severity of skin lesions, complications and determined on the basis of individual differences, and the exclusion of a hormone contraindieations, to quadruple therapy, 38 cases were cured （ 59% ）, effective in 17 cases （ 27% ）, effective in 8 cases （ 13% ）, 1 case of death. The total effeetive rate was 86%. In 64 patients in the treatment, CD3, CD4 and CD4/ CD8 ratio were deereased, CD8 increased in different degrees, after treatment （ except 1 cases dead ）, CD3 and CD4 were significantly increased （ P〈0.05 ）, CD8 decreased, but there was no significant difference compared with that before （ P〉0.05 ）. Conclusion. Integrating traditional Chinese and Western medicine therapy in the treatment of facial herpes Zoster ean effeetively reduce inflammation, regulate cell immunity, prevent the toxicity and damage to the nerves and reduce the occurrence of postherpetic neuralgia.
