中药穴位熨敷治疗脑外伤后头痛的临床疗效观察

摘　　要：目的:评估中药穴位熨敷治疗脑外伤后头痛的临床疗效与安全性,为丰富中药穴位熨敷的临床治疗范围提供科学依据,为脑外伤后头痛提供一种安全有效的治疗方法。方法:采用随机对照试验的设计方案进行临床观察。根据拟定的诊断、纳入、排除标准,于2012年10月-2013年5月,确定了90例脑外伤后头痛患者,随机分为两组。治疗组45例为中药穴位熨敷治疗组,对照组为西医止痛药口服治疗组。然后观察患者的疗效。结果:经研究所得,初诊治疗组与对照组疼痛评分组间对比均无显著性差异(P<0.05),具有可比性;治疗组在治疗后3d、7d、2周分别为(6.33±1.48)、(4.91±1.50)、(2.31±1.47),对照组分别为(8.47±1.58)、(6.32±1.64)、(3.92±1.55),三阶段疼痛评分差异均有统计学意义(P<0.05);治疗组显效、有效、无效例数分别为36、24、1,对照组分别为23、31、6,两组疗效比较有统计学意义(P=0.01);两组治疗前后血、尿、便常规,肝、肾功能,均无明显变化。治疗组及对照组病例均未出现明显药疹、皮肤瘙痒等不良反应。结论:中药穴位熨敷治疗脑外伤后头痛疗效显著,副作用小,值得推广。To evaluate clinical efficacy of TCM acupoint fomentation on headache following brain injury. Methods：90 patients were randomly divided into the treatment group and the control group, 45 patients in each. The treatment was given TCM acupoint fomentation, the control took orally painkiller. Results：Before treatment, pain scores in both were compared, the difference did not show statistical significance （P〉0.05）. Pain scores in the treatment were （6.33±1.48）, （4.91±1.50） and （2.31±1.47）, in the control were （8.47± 1.58）, （6.32±1.64） and （3.92±1.55） in 3d, 7d and 2 weeks of treatment; the difference was statistically significant （P〉0.05）. In the treatment, 36 patients were markedly effective, 24 for effective and 1 for invalid;in the control, 23 patients were markedly effective, 31 for effective and 6 for invalid, the difference was statistically significant （P=0.01）. There was no significant difference in blood routine, urine routine and stool routine before and after treatment, and significant eruption and pruritus. Conclusion： TCM acupoint fomentation was effective on headache following brain injury, worthy of clinical promotion.
