存活素在类风湿关节炎滑膜组织中的表达及意义

摘　　要：目的通过检测RA患者滑膜组织中存活素表达水平，分析其与RA患者临床特征之间的关系，并探讨存活素在RA发病机制中的作用。方法用免疫组织化学的方法检测65例RA患者，9例OA患者，21份对照组滑膜组织中存活素表达水平，RA患者根据DAS28分为活动组与缓解组。记录入组患者临床资料：性别、年龄、病程、ESR、CRP、RF、CCP、DAS28评分。检测各组存活素蛋白的表达情况，并分析RA活动组存活素蛋白表达与其临床病理参数的相关性，多组间均数的比较使用单因素方差分析，率的比较采用Pearson矿检验或似然比矿检验，各指标与临床指标的相关性分析采用Spearman相关分析。结果①RA患者组滑膜组织中存活素表达水平明显高于OA组及正常对照组（砖O．001）。②RA活动组患者滑膜组织中存活素表达水平明显高于缓解组x^2=41．62，P=0．001）。③RA活动组患者存活素表达水平与ESR、CRP、DAS28评分呈正相关（r值分别为：0．297、0．411、0．416，P＜0．05），而与CCP、RF、性别、年龄、病程无相关性（r值分别为：0．004、-0．114、-0．074、-0．035、-0．188，P＞0．05）。结论存活素在RA滑膜组织中表达水平增高提示了其与滑膜成纤维细胞的凋亡异常相关，参与了RA的发病过程，检测存活素水平有助于对RA疾病活动性的评估。Objective To investigate the role of survivin in the pathogenesis of rheumatoid arthritis （RA） by detecting of expression levels of survivin in synovial tissue of patients with rheumatoid arthritis and analyze its relationship with clinical features of RA patients. Methods The expression levels of survivin in synovial tissue of patients with RA were detected by immunohistochemistry method in 65 cases of RA, 9 cases of osteoarthritis, 21 normal controls. According to the＇ disease activity score （DAS28）, RA patients were divided into active disease group and remission group. Clinical data of these patients were recorded, including gender, age, course of disease, erythrocyte sedimentation rate （ESR）, C reactive protein （CRP）, rheumatoid factor （RF）, anti-cyclic citrullinated peptide antibodies （CCP） and DAS28 score. Comparison of means of above parameters in multiple groups was conducted with variance analysis. Pair-wise comparisons among groups were conducted with one-way ANOVA. The expression of survivin was examined and compared among the three groups, the relationship between survivin and the clinical pathological features of the active group of RA were analyzed. The likelihood ratio chi-square test or Pearson chi-square test was used in the comparison of rates, Spearman＇s correlation analysis was used among parameters in the study. Results① The expression of survivin in synovial tissue of patients with RA was significantly higher than that in OA group and normal control group （P=0.001）. ② The expression of survivin in synovial tissue of active RA group was significantly higher than in the remission group （X^2=41.62, P=0.001 ）. ③ The survivin expression level was positively correlated with ESR, CRP, DAS28 score （r=0.297, 0.411, 0.416 respectively, P〈0.05）, but not correlatedwith anti-CCP, RF, sex, patients＇ age and course of disease in RA patients （r=0.004, -0.114, -0.074, -0.035, -0.188 respectively, P〉0.05）.
