摘 要:目的:定量研究下颌偏斜患者双侧髁突形态学特征与软骨下骨骨质变化,探讨双侧髁突的改建机制。方法:对55名下颌偏斜患者进行临床资料采集和CT扫描,利用Mimics软件重建下颌骨三维模型,分别测量双侧髁突表面积、体积、表面曲率及软骨下骨骨密度,采用配对t检验、Bonferroni法及完全随机区组设计的方差分析对结果进行统计学分析。结果:偏斜侧髁突表面积与体积(表面积:1666.14±318.3 mm2;体积:1981.5±418.3 mm3)均大于非偏斜侧(表面积:1151.5±134.7 mm2;体积:1460.3±165.0 mm3)(P<0.001);偏斜侧髁突后斜面曲率(0.39±0.08)大于非偏斜侧(0.26±0.07)(P<0.001);偏斜侧软骨下骨骨密度(前斜面:523.01±118.1;后斜面:549.07±120.6 HU)大于非偏斜侧(前斜面:497.02±116.2 HU;后斜面:485.30±148.4 HU)。结论:下颌不对称运动造成髁突表面压力不均衡,从而导致双侧髁突表面形态及软骨下骨骨密度发生非对称改建。Objective:To explore the remodeling mechanism of bilateral condyles by investigating morphological characteristics and subchondral bone density of bilateral condyles in patients with mandibular deviation.Method:Clinical data and computed tomography images of 55 patients with mandibular deviation were collected.Three dimensional model of mandibular were constructed by mimics and the condylar volume,surface size,surface curvature and subchondral bone density of bilateral condyles were analyzed.Paired t-test,Bonferroni and ANOVA of Randomized Complete Block-design were performed to analyze the statistical difference.Result:The condylar volume and surface size on deviated side (surface size:(1666.14±318.3) mm2,volume:(1981.5±418.3) mm3 were significantly larger than those of non-deviated side (surface size:(1151.5±134.7) mm2,volume:(1460.3±165.0)mm3 (P <0.001).The mean Gaussian curvature of posterior slope of condyles on deviated condyle (0.39±0.08) were larger than those of non-deviated condyle (0.26±0.07) (P <0.001).The subchondral bone density on deviated condyle (anterior slope:523.01±118.1 HU;posterior slope:549.07±120.6 HU) were lager on those of non-deviated side (anterior slope:497.02±116.2 HU; posterior slope:485.30±148.4 HU).Conclusion:Unbalanced stress on bilateral condyles caused by mandibular asymmetric movement results in asymmetric remodeling of bilateral condylar surface morpholoyg and subchondral bone density.
