耳郭外伤的急诊处理与美容修复

摘　　要：目的 总结外伤性耳郭的急诊处理与美容修复.方法 回顾性总结2012年1月至2014年1月,我科治疗的21例耳郭严重外伤及外伤后继发畸形患者的临床资料.依据伤情不同,采用不同的处理方式进行修复.结果 清创后直接缝合9例,全厚皮片移植7例,各种皮瓣修复5例;创面均Ⅰ期愈合,外观良好,无明显瘢痕、错位与变形.其中耳郭部分缺失9例,需行二次整复治疗.结论 对于严重耳郭外伤的急诊处理与美容修复,应根据耳郭致伤原因、缺损部位、严重程度,遵照“创伤美容修复”理念,应用整形外科的微创技术与美学理论,灵活掌握适应证,实施及时准确的治疗,以达到美容修复的目的.Objective To summarize emergency treatment and cosmetic repair of the severe auricular trauma.Methods From Januay 2012 to Januay 2014,retrospective summay was performed on the 21 cases with the severe auricular trauma and seconday deformity.According to different traumatic conditions,the different operations were adopted.Results Of 21 cases,9 cases were treated with the simple debridement and stitching,7 cases were repaired with full-thickness skin grafting flap and 5 cases with different kinds of flaps.The wound healed primarily and the good appearances obtained without obvious scars,dislocation and deformation.Among them,9 cases with partauricle defect needed seconday repair.Conclusion According to idea of cosmetic repair of trauma,emergency treatment and cosmetic repair of severe auricular trauma should be treated on the basis of causes of injuries,the location,size,defective shape,application of minimally invasive technology and aesthetic theory and handle of indications to perform the treatments promptly and exactly.
