老年髋部骨折行手术治疗患者住院日影响因素分析

摘　　要：目的探讨影响老年髋部骨折行手术治疗患者住院日的影响因素。方法自2010-02—2012-02,纳入采用人工全髋关节置换术或人工股骨头置换术治疗年龄≥70岁的髋部骨折共91例。分析的因素包括患者的性别、年龄、伤侧、骨折类型、受伤至入院时间、受伤至手术时间、入院至手术时间、手术方式、术前合并症、入院血清白蛋白含量、入院淋巴细胞计数、入院血红蛋白含量、入院白细胞计数。结果单因素分析显示入院至手术时间、入院血红蛋白及骨折类型对患者住院时间有明显影响,多因素分析显示入院至手术时间延长和入院血红蛋白减少均显著增加了患者住院日(P=0.002和P=0.01)。结论入院至手术时间延长以及贫血会增加髋部骨折行手术治疗患者的住院日。Objective To investigate the factors related to the length of stay（LOS） in elderly patients with hip fracture. Methods From Feb. 2010 to Feb. 2012, ninety-one elderly patients（age≥70 years） with hip fractures underwent either total hip arthroplasty or bipolar arthroplasty. The factors investigated were gender, age, side of fracture （left or right）, the type of fracture （femoral neck or trochanteric region）, the time interval from fracture to admission, the time interval from fracture to surgery, the time interval from admission to surgery, type of operation（total hip arthroplasty or hemiarthroplasty）, comorbidities, admission serum albumin, admission total lymphocyte count, admission hemoglobin, admission white blood cells. Results For the ninty one study patients, on univariable analysis, three factors were significantly associated with prolonged LOS： increased time interval from admission to surgery, decreased admission hemoglobin and trochanteric region. On multivariable linear regression analysis, increased time interval from admission to surgery（P =0.002） and decreased admission hemoglobin（P =0.01） were significantly associated with increased LOS. Conclusion Elderly patients with hip fractures with an increased time interval from admission to surgery or decreased admission hemoglobin have a longer hospital LOS.
