摘 要:目的 通过Meta分析评估PET/CT诊断非小细胞肺癌纵隔淋巴结分期的准确性.资料与方法 通过Cochrane Library、PubMed、中国生物医学光盘数据库(CBMdisc)、中国期刊全文数据库(CNKI)、万方和维普等数据库收集2000~2011年已发表的有关PET/CT诊断非小细胞肺癌纵隔淋巴结分期的文献,按预设标准对文献进行筛选和质量评价.使用Meta-Disc软件进行Meta分析,绘制汇总受试者工作特征曲线,计算曲线下面积(AUC)和Q*指数.结果 符合纳入标准的文献共27篇(3667例患者).以患者为研究对象,诊断的灵敏度和特异度分别为0.78 (95% CI0.76~0.81)、0.87 (95% CI 0.85~0.88),AUC和Q*指数分别为0.8869、0.8175.以淋巴结为研究对象,诊断的灵敏度和特异度分别为0.66(95%CI0.64~0.69)、0.94(95%CI 0.94~0.95),AUC和Q*指数分别为0.9197、0.8530.结论 PET/CT诊断非小细胞肺癌纵隔淋巴结分期的准确度高,具有较大的临床应用价值,但其灵敏度有待进一步提高.Purpose To assess the accuracy of mediastinal lymph node staging in non-small cell lung cancer using PET/CT by Meta-analysis.Materials and Methods Literatures on mediastinal lymph nodes staging of non-small cell lung cancer literature by PET/CT published from 2000 to 2011 were filtered and quality-assessed according to preset criteria from the Cochrane Library,PubMed,Chinese Biomedical CD library (CBMdisc),CNKI,Wanfang database,VIP and other database.Meta-analysis was performed using Meta-Disc software,and receiver operating characteristic curve was applied,area under the curve (AUC) and Q* index were calculated.Results 27 relevant literatures were in accordance with the inclusion criteria (3667 patients).The diagnostic sensitivity and specificity based on the patients were 0.78 (95% CI 0.76-0.81) and 0.87 (95% CI 0.85-0.88),AUC and Q* index were 0.8869 and 0.8175.The diagnostic sensitivity and specificity based on lymph nodes were 0.66 (95% CI 0.64-0.69) and 0.94 (95% CI 0.94-0.95),AUC and Q* index were 0.9197 and 0.8530.Conclusion PET/CT has a high accuracy for the mediastinal lymph node staging in non-small cell lung cancer with high clinical applicable value,but its sensitivity needs to be improved.
