摘  要:目的:用中医体质分类方法,探讨痰湿质高血压病患者心血管高危风险的特征,并寻找这些高危风险因素在遗传方面的生物标志。方法:纳入506例高血压病患者(痰湿质组261例,非痰湿质组245例),以左心室重量指数(LVMI)和颈动脉内膜中层厚度(IMT)为指标,对LVMI和颈动脉IMT与脂联素基因4个单核苷酸(SNP)多态性进行相关分析。结果:痰湿质组高血压病患者的LVMI为(160.13±5.29)g/m2,非痰湿质组为(143.24±5.46)g/m2,两组比较差异有统计学意义(P=0.029);痰湿质组的颈动脉IMT为(0.66±0.01)mm,非痰湿质组为(0.63±0.01)mm,两组比较差异有统计学意义(P=0.021)。痰湿质组高血压病患者的LVMI在rs182052各型的水平分别为A型(141.87±14.21)g/m2、G型(188.17±10.65)g/m2、AG型(176.22±10.35)g/m2,表现出G型的LVMI最高,其差异有统计学意义(P=0.035)。结论:1痰湿质高血压病患者较非痰湿质高血压病患者存在左心室肥厚程度严重、颈动脉内膜厚度增加等靶器官损害的倾向;2脂联素基因rs182052 G型可能是痰湿质高血压病患者心血管高危风险的遗传标志之一。Objective: To explore the character of high cardiovascular risks in hypertensive patients of dampness-phlegm constitution( DPC) based on constitution classification of traditional Chinese medicine,and seek the genetic markers of these high risks. Methods: A total of 506 hypertensive patients were involved,among which 261 cases were DPC and 245 cases were non-DPC. The left ventricular mass index( LVMI) and carotid intima-media thickness( IMT) were measured,and the correlation between LVMI and carotid IMT and 4 single nucleotide polymorphisms( SNP) in adiponectin gene was analyzed.Results: The LVMI was( 160. 13 ± 5. 29) g /m^2 in DPC group and was( 143. 24 ± 5. 46) g /m^2 in non-DPC group,with a statistical difference between the two groups( P = 0. 029); the carotid IMT was( 0. 66 ± 0. 01) mm in DPC group and(0.63 ±0.01) mm in non-DPC group,with a statistical difference between the two groups( P =0.021). In the DPC group,the LVMIs of patients with rs182052 A,rs182052 G and rs182052 AG genotypes were( 141. 87 ± 14. 21) g /m^2,( 188. 17 ±10. 65) g /m^2and( 176. 22 ± 10. 35) g /m^2 respectively,and the LVMI was the highest in patients with rs182052 G genotype( P =0.035). Conclusion: The conditions of LVMI and carotid IMT in hypertensive patients with DPC was worse than patients with non-DPC and the rs182052 G in adiponectin gene may be one of the genetic markers of high cardiovascular risk in hypertensive patients with DPC.
