摘  要:目的:探讨脂肪溶解液联合超声剪在乳腺癌腋淋巴结清扫术中的护理配合。方法2012年1月~2012年11月本科完成改良乳腺癌根治术173例,分为溶脂超声剪组(89例)和传统组(84例)。溶脂超声剪组使用脂肪溶解液联合超声剪,传统组采用超声刀加电刀。比较2组的总手术时间、手术出血量、术后引流量和拔管时间及手术并发症的发生情况。结果溶脂超声剪组的总手术时间、术中出血量、术后引流量、引流管留置时间和术后患侧上肢水肿情况均优于传统组(P〈0.05),2组术后均无大出血病例。结论用脂肪溶解液联合超声剪行乳腺癌腋淋巴结清扫术安全可靠,具有缩短手术时间、减少术后引流量、减少损伤的作用。术前充分准备,术中手术配合熟练,是保证手术顺利完成的必备条件。Objective To study the nursing of fat-soluble liquid combined with ultrasonic shear in axillary lymph node dissection for breast cancer. Methods 173 patients with breast cancer underwent modified radical mastectomy, in which 89 cases were operated by fat-soluble liquid combined with ultrasonic shear as the fat-dissolve group and 84 cases were operated by ultrasonically activated scalpel as the traditional group. The total operative time, intraoperative blood loss, postoperative drainage quantity, tube drawing time and postoperative complication would be compared between these 2 groups. Results The total operative time, intraoperative blood loss, postoperative drainage quantity, tube drawing time and postoperative upper extremity edema of the fat-dissolve group were less than the traditional group(P〈0.05).There was no postoperative hemorrhage in both groups. Conclusion Fat-soluble liquid combined with ul-trasonic shear in axillary lymph node dissection for breast cancer is safe, with shorter operative time and reduced postoperative drainage quantity and damage. Thorough preoperative preparation and proficiency in usage of the instrument during surgery is a pre-requisite to ensure the successful completion of surgery.
