清瘟败毒饮对脓毒症急性肾损伤患者Cys-C、KIM-1与NGAL表达的影响

摘　　要：目的观察清瘟败毒饮对脓毒症急性肾损伤(AKI)患者肾功能的影响。方法将脓毒症AKI患者随机分为治疗组与对照组,并给予常规西药治疗;治疗组同时予清瘟败毒饮口服或鼻饲治疗,疗程7天。治疗前后进行APACHEⅡ评分,测定血清C反应蛋白(CRP)、尿素氮(BUN)、肌酐(Scr)、胱抑素C(Cys-C)、尿肾损伤分子1(KIM-1)、人中性粒细胞明胶酶相关脂质运载蛋白(NGAL)水平变化情况。结果治疗后两组APACHEⅡ评分、CRP水平显著下降(P<0.05),且治疗组低于对照组(P<0.05)。治疗后两组BUN、Scr水平均升高(P<0.05),但治疗组低于对照组(P<0.05)。治疗后两组KIM-1、NGAL显著下降(P<0.05),且治疗组低于对照组(P<0.05);治疗后两组Cys-C上升(P<0.05),且治疗组低于对照组(P<0.05)。结论清瘟败毒饮能减少脓毒症AKI患者Cys-C分泌,下调KIM-1、NGAL表达以保护肾功能。Objective To observe the effect of＂Qingwen Baidu Drink＂on renal tubule function in acute kidney injury（AKI） patients with sepsis. Methods The AKI patients with sepsis were randomly divided into treatment group and control group. Both of the groups were treated by routine therapy,and cases in treatment group were treated by＂Qingwen Baidu Drink＂or nasogastric gavage,with the course of 7 days. The APACHE II score and levels of CRP,BUN,Scr,Cys-C,KIM-1,and NGAL were detected before and after treatments. Results After treatment the APACHE Ⅱ score and CRP level were markedly decreased（P〈0.05）,with significant differences between the treatment group and the control group（P〈0.05）. After treatment,the levels of BUN,Scr and Cys-C in both groups were increased（P〈0. 05）,but the levels of KIM-1 and NGAL were reduced both groups（P〈0. 05）,with significant differences between the two groups（P〈0. 05）. Conclusion ＂Qingwen Baidu Drink＂can inhibit the Cys-C secretion in AKI patients with sepsis,down-regulate the expressions of KIM-1and NGAL to protect the renal tubule function.
