三痹汤熏洗结合关节镜下微创手术治疗下肢骨折术后膝关节僵直的临床观察

摘　　要：目的观察三痹汤熏洗结合关节镜下微创手术治疗下肢骨折术后膝关节僵直的临床疗效。方法将62例下肢骨折术后膝关节僵直的患者随机分为治疗组与治疗组,每组31例。对照组予关节镜下伸膝装置松解手术及术后功能锻炼,治疗组同时加用中药三痹汤熏洗。两组疗程均为8周,观察膝关节功能恢复情况,以及治疗后、治疗后6个月的膝关节屈曲活动度变化情况。结果 1治疗组、对照组膝关节功能恢复优良率分别为87.10%和70.97%;组间功能恢复疗效比较,差异有统计学意义(P<0.05)。2治疗前后组内比较,两组膝关节屈曲活动度差异均有统计学意义(P<0.05);组间治疗后比较,膝关节屈曲活动度差异有统计学意义,治疗组优于对照组(P<0.05);组间治疗后6个月比较,膝关节屈曲活动度差异有统计学意义,治疗组优于对照组(P<0.05)。结论三痹汤熏洗结合关节镜下微创手术治疗下肢骨折术后膝关节僵直,可显著恢复膝关节活动功能,且远期疗效较佳。Objective To observe the clinical efficacy of ＂Sanbi Lotion＂ and minimally invasive surgery in treating knee ankylosis of post-operative fracture of lower limb. Methods Sixty-two cases were randomized into treatment group and control group, with 31 cases in each group. The control group was treated by lysis of knee extension apparatus with arthrescope and functional exercise, and the treatment group was added with ＂Sanbi Lotion＂, with the course of 8 weeks. We observed the functional recovery of knee-joint and compared the flexion before and after treatment. Results ① The effective rate of functional recovery was 87.10% in the treatment group and 70.97% in the control group, with a difference between the two groups （ P 〈 0.05 ）. ② There was significant difference in flexibility between before and after treatment in both groups （ P 〈 0. 05 ）, after treatment, significant difference was found between the two groups （ P 〈 0.05 ） ; 6 months after treatment, with a significant difference between the two groups （ P 〈 0.05 ）. Conclusion Combined ＂ Sanbi Lotion＂ and minimally invasive surgery has positive effect on the knee function and long-term curative effect in patients with post-operative knee ankylosis.
