腹膜透析对终末期肾脏病患者细胞免疫及营养状况的影响

摘　　要：目的观察终末期肾脏病(ESRD)患者细胞免疫和营养状况,及腹透治疗对其的影响。方法选择2010年7月至2013年6月的ESRD患者40例,同期健康体检者40例。其中ESRD患者在CAPD治疗前和治疗6个月后分别检测,检测项目为T细胞亚群、体质量指数、上臂肌围、血浆白蛋白、瘦素水平。结果 1)与对照组比较,ESRD患者CD3+、CD4+、CD4+/CD8+均降低(P<0.05);ESRD患者CAPD治疗后,CD3+、CD4+、CD4+/CD8+均升高(P<0.05);2)与对照组比较,ESRD患者血浆白蛋白明显下降(P<0.01);瘦素水平明显上升(P<0.01)。ESRD患者CAPD治疗后,血浆白蛋白明显上升(P<0.01),瘦素水平下降(P<0.05)。结论 ESRD患者存在细胞免疫缺陷和营养不良,CAPD治疗可改善ESRD患者细胞免疫功能及营养状况。Objective To observe the cellular immunity and nutritional status of patients with end-stage renal disease （ESRD） and the influence of CAPD treatment on them. Methods 40 cases of patients with ESRD from July 2010 to June 2013 and 40 cases of healthy persons for the same period were selected. Patients with ESRD were respectively tested before CAPD treatment and 6 months after treatment, the testing items were T cell subsets, body mass index, mid-arm muscle circumference, plasma albumin and the level of leptin. Results 1） Compared with the control group, the CD3＋ , CD4＋ and CD4＋/ CD8＋ of patients with ESRD were all decreased （P〈0. 05） ; after CAPD treatment, the CD3＋ , CD4＋ and CD4＋/CD8＋ of patients with ESRD were all increased （P〈0. 05）; 2） Compared with the control group, the plasma albumin of patients with ESRD was significantly decreased （P〈0. 01） ; the level of leptin was significantly increased （P〈0.01） ; after CAPD treatment, the plasma albumin of patients with ESRD was significantly increased （P〈0. 01）, the level of leptin was decreased （P〈0.05）. Conclusion Patients with ESRD have cellular immune deficiency and malnutrition, while CAPD treatment can improve the cellular immune function and nutritional status of patients with ESRD.
