改良式会阴侧切缝合术的临床应用

摘　　要：目的探讨自然分娩过程中采用改良式会阴侧切术缝合方法的临床价值。方法将2013年1月～2013年11月在本院生产过程中须施行会阴侧切术的初产妇326例，随机分为观察组和对照组各163倒。观察组施行改良式会阴侧切，对阴道黏膜、会阴肌层、皮下脂肪与皮内采用连续缝合法进行缝合；对照组施行传统式会阴侧切，对阴道黏膜、会阴肌层与皮下脂肪采用间断缝合法实行缝合后，最后皮内连续缝合。分析两组患者的临床资料，比较缝合时间的长短、伤口出血量的多少以及伤口重度疼痛发生率。结果观察组会阴切口出血量明显少于对照组，组间差异有统计学意义（P＜0．05）；缝合时间较对照组明显缩短，两组间差异有统计学意义（P＜0．05）；重度疼痛发生率降低，切口炎性反应轻，和对照组比较，差异有统计学意义（均P＜O．05）。结论改良式会阴侧切连续缝合法操作简单，切口愈合良好，能缩短住院时间，有利于产妇产后身心健康，具有重要的临床应用价值，值得临床推广应用。Objective To investigate the effect of natural childbirth process using modified lateral perineal incision, suture method on the healing of the perineal wound. Methods 326 cases of primipara were randomly divided into observation group and control group. The observation group received modified type of lateral episiotomy, the vaginal mucosa, perineal muscle, subcutaneous fat and skin with continuous suture to suture. The control group received traditional episiotomy, the vaginal mucosa, muscle layer and subcutaneous fat perineal with interrupted suture to suture, the intracutaneous continuous suture. The suture time, incidence of severe pain and wound bleeding amount were observed. Results The observation group was treated with modified lateral episiotomy was performed after continuous suture. The perineal hleeding volume of observation group was less than that of the control group. The difference was statistically significant between the two groups （P〈Z0.05）. The suture time of observation group was obviously shortened that that of the control group （P〈0.05）. The incidence of severe pain, wound inflammation reaction to light of observation group were statistically significant difference from that of the control group （P〈0.05）. Conclustion Improved episiotomy suture is a simple operation, which has good wound healing and shortens the hospitalization time.
