摘  要:对从临床分离的铜绿假单胞菌进行耐药性分析及质粒分析,为临床合理使用抗生素提供依据。对临床收集的56株铜绿假单胞菌,通过K-B纸片扩散法进行耐药性测定;并通过碘淀粉法和酸法测定β-内酰胺酶;提取质粒进行琼脂糖电泳鉴定。临床分离株对头孢西丁、头孢呋辛、呋喃妥因、利福平、万古霉素等的耐药性达到了100%,对卡那霉素、红霉素、磺胺甲噁唑/甲氧苄啶、复方新诺明、头孢噻肟、头孢哌酮、阿奇霉素、头孢曲松、哌拉西林等抗生素耐药性介于62.5%~96.4%之间,环丙沙星、头孢哌酮/舒巴坦、美罗培南耐药性较低介于21.4%~25%之间,铜绿假单胞菌对多黏菌素耐药性最低,仅为1.8%;通过碘淀粉法和酸法,56株中有37株β-内酰胺酶阳性菌株,占总菌株的66%;质粒鉴定结果显示,所有菌株均含有1个大约为20 kb的质粒。铜绿假单胞菌临床分离株具有较广的耐药谱,且多数能产β-内酰胺酶;所有菌株都含1个20 kb的大型质粒,可能与其耐药性相关,提示消除质粒可能降低其耐药性。To analyze plasmids profile and antibiotic resistance of Pseudomonas aeruginosa from clinical samples in Zhejiang's hospital, the work provides a basis for clinical and rational use of antibiotics. K-B paper diffusion method was used to detect antibi- otics sensitivity of the 56 P. aeruginosa isolates to commonly used antibiotics. In addition, 12-starch method and the acid titration assay were used to analyze Beta lactamase expressed by these P. aeruginosa isolates. Plasmids from these P. aeruginosa isolates were extracted and analyzed by agarose gel electrophoresis. Antibiotic resistance of clinical isolates for cefoxitin, cefuroxime, nitrofurantoin, rifampin and vancomycin reached 100%. The antibiotic resistance of clinical isolates for kanamycin, erythromycin, SXT, SMZ-TMP, cefotaxime, cefoperazone, azithromycin, ceftriaxone and piperacillin are from 62.5 % to 96.4%. The antibiotic resistance clinical isolates for ciprofloxacin,cefoperazone/sulbactam, meropenem are low and within a range of 21.4% to 25%. The clinical isolates displayed the lowest antibiotic resistance to polymyxin B with a value of only 1.8%. By the 12-starch method and the acid titration assay, it was defined that 37 out of the 56 clinical strains are beta-lactamase positive, accounting for 66% of the total strains. It was shown that all the 56 strains of P. aeruginosa contained a plasmid with a molecular size of about 20 kb. P. aeruginosa clinical isolates have a broad spectrum of antibiotic resistance, and most of them can produce beta lactamase. All strains contain a large plasmid that is 20 kb. This plasmid may be related to its antibiotic resistance. It is possible that the P. aeruginosa antibiotic resistance can be reduced by removing the plasmid.
