通心络联合氯吡格雷对不稳定型心绞痛患者血管内皮功能和炎性因子的影响

摘　　要：目的探讨通心络胶囊联合硫酸氢氯吡格雷对不稳定型心绞痛患者血管内功能和炎性因子的影响。方法 80例不稳定型心绞痛患者随机分为对照组、研究组各40例。2组均常规应用氯吡格雷、低分子肝素钙、钙离子拮抗剂、β受体阻滞剂等冠心病的规范治疗;研究组在规范治疗基础上加服通心络胶囊3粒,3次/d。检测治疗前及治疗4、8周后2组患者一氧化氮(NO)、内皮素-1(ET-1)、肿瘤坏死因子-α(TNF-α)、白介素-1(IL-1)、白介素-6(IL-6)水平变化。结果 2组患者治疗前NO、ET-1、TNF-α、IL-1、IL-6水平比较差异均无统计学意义(P>0.05)。与治疗前比较,2组患者治疗4、8周后NO均明显升高(P<0.05),ET-1、TNF-α、IL-1、IL-6均明显降低(P<0.05)。与对照组比较,研究组治疗后4、8周后NO水平明显升高(P<0.05),ET-1、TNF-α、IL-1、IL-6水平明显降低(P<0.05)。治疗4、8周后研究组总有效率明显高于对照组(分别为90.0%、95.0%和72.5%、77.5%,P<0.05)。结论通心络胶囊联合硫酸氢氯吡格雷能明显改善不稳定型心绞痛患者血管内皮功能,提高不稳定型心绞痛临床治疗效果。Objective To investigate the effect of Tongxinluo capsule combined with clopidogrel on vascular function and inflammatory factors in patients with unstable angina .Methods Eighty cases with unstable angina were randomly divided into control group and study group with 40 cases in each group .Two groups were treated with conventional therapy with clopi-dogrel and low molecular weight heparin calcium , calcium antagonists , beta blockers , coronary heart disease; study group based on the standard treatment , added Tongxinluo capsule 3 capsules, 3 times /d.Nitric oxide （NO） and endothelin 1 （ET-1）, tumor necrosis factor-α（TNF-α）, interleukin-1 （IL-1）, interleukin-6 （IL-6） levels were tested before treatment and 4, 8 weeks after treatment in the 2 groups of patients.Results The levels of NO, ET-1, TNF-α, IL-1 and IL-6 were revealed no significant different before treatment between the 2 groups of patients （ P 〉0.05）.Compared with before treatment, 4 and 8 weeks after treatment, 2 groups of patients’ NO were significantly increased （ P 〈0.05）, ET-1, TNF-α, IL-1, IL-6 were decreased （ P 〈0.05）.Compared with the control group, NO was significantly increased in the study group after 4 and 8 weeks’ treatment （ P 〈0.05）, ET-1, TNF-α, IL-1, IL-6 levels were significantly decreased （ P 〈0.05）.After 4 and 8 weeks treatment , the total effectiveness rate in the study group was better than that in the control group （ which were 90 .0%, 95.0%and 72.5%,77.5%respectively, P 〈0.05）.Conclusion It demonstrated that the Tongxinluo capsule combined with clopidogrel hydrogen sulfate can obviously improve the vascular endothelial function in patients with unstable angina ;im-prove the clinical treatment effect on unstable angina pectoris .
