摘 要:目的探讨超重与肥胖儿童的血清甲状腺激素及瘦素(LP)水平变化情况,为超重肥胖儿童的健康指导和临床干预提供依据。方法将161名学龄儿童根据体质量指数(MBI)分为体重正常组(82名)、超重组(43名)和肥胖组(36名),放射免疫分析法检测三组儿童的血清FT3,FT4,TSH与LP水平,并分析三组间的差异,Pearson相关分析BMI,FT3,FT4,TSH,LP之间的相关性。结果男性与女性中不同体重组间FT4,TSH和LP差异均有统计学意义(P值均<0.05),肥胖组FT4水平较低,TSH,LP水平较高。女性中不同体重组间FT3水平差异有统计学意义,肥胖组较高;体重正常男性和女性组BMI与血清LP之间存在有统计学意义的正相关(r值分别为0.844,0.707,P值均<0.01),而BMI与血清FT3,FT4,TSH之间及血清FT3,FT4,TSH,LP之间相关无统计学意义;超重与肥胖男性组BMI与血清TSH,LP有统计学意义的正相关(r值分别为0.735,0.701,P值均<0.01),BMI与FT4水平呈现负相关(r=-0.279,P=0.043),血清TSH与LP水平呈正相关(r=0.685,P<0.01)。超重与肥胖女性组BMI与血清TSH,LP有统计学意义的正相关(r值分别为0.767,0.442,P值分别为0.000,0.024),与FT3,FT4呈负相关(r值分别为-0.549,-0.467,P值分别为0.004,0.016),FT3与LP存在正相关(r=0.486,P=0.012)。结论肥胖对儿童甲状腺轴功能有潜在影响,且影响程度与超重或肥胖的程度相关,瘦素可能是这一过程中的重要调节因子。Objective To investigate serum thyroid hormone and leptin (LP) in overweight and obese children. Methods One hundred and sixty one school-aged children were divided into three groups according to the body mass index (BMI) : normal weight ( n = 82 ), overweight ( n = 43 ) and obese ( n = 36 ). Radioimmunoassay was used to detect the levels of serum FT3, FT4, TSH and leptin ( LP ). One-way ANOVA and Pearson correlation analysis were used in data analysis. Results There were signifi- cant differences of FT4, TSH and LP among three groups both in boys and girls( P〈0.05 ). Obese children had lowest level of FT4 and highest level of TSH and LP. In girls, FT3 differed significantly among three BMI groups. In normal-weight group, beth boys and girls, significant positive correlation was found between BMI and serum LP (rm= 0.844, Pm= 0.000 ;r f= 0.707 ,Pf= 0.000}. But no statistically significant correlation existed between BMI and FT3, FI'4 and TSH, and among FT3, FT4, TSH and LP. In male overweight and obese group, there was positive correlation between BMI and serum TSH, LP( r was 0.735 and 0.701, P values was 0.000 and 0.000, respectively . There was weak negative correlation between BMI and FT4 ( r = - 0.279, P = 0.043 ) and positive correlation between TSH and LP (r= 0.685 ,P= 0.000 ). In female overweight and obese group, BMI was positively correlated with TSH,LP( r was 0.767 and 0.442, P values was 0.000 and 0.024, respectively) and negatively correlated with FT3, FT4( r was -0.549 and -0.467, and P values was 0.004 and 0.016, respectively), Positive correlation existed between FT3 and LP ( r= 0.486,P=0.012). Conclusion Obesity could interfere with thyroid function in children. Leptin may be an important regulatory factor in thyroid dysfunction among obese children.
