幽门螺杆菌感染对老年高血压患者颈动脉粥样硬化的影响

摘　　要：目的研究老年高血压患者幽门螺杆菌(Hp)感染情况及动脉粥样硬化相关危险因子,探索Hp感染在老年高血压患者颈动脉粥样硬化发生、发展过程中的作用。方法选取老年高血压病患者83例,检测其Hp感染状况及血清低密度脂蛋白胆固醇(LDL-C)、脂蛋白(a)[Lp(a)]、同型半胱氨酸(Hcy)和高敏C反应蛋白(hs-CRP)含量,同时进行颈动脉超声检测,以Hp感染状况分组,对检测结果进行统计学分析。结果共检测Hp阳性37例,阳性率达44.6%,Hp阳性组患者颈动脉内膜中层厚度(CIMT)异常率及颈动脉形成斑块率均高于Hp阴性组(P<0.05);Hp阳性组LDL-C、Lp(a)、Hcy和hs-CRP 4项血清学指标均高于Hp阴性组(P<0.05);Hp现症感染组LDL-C、Hcy和hs-CRP 3项指标高于既往感染组(P<0.05)。结论老年高血压患者中Hp感染与颈动脉粥样硬化关系密切,Hp感染能增加高血压患者颈动脉粥样硬化的风险。Objective To investigate the status of Helicobacter pylori( Hp) infection and atherosclerosis risk factors in elderly patients with hypertension,and to investigate the role of Hp infection on the occurrence and development of carotid atherosclerosis in elderly patients with hypertension. Methods The status of Hp infection and the serum levels of low-density lipoprotein cholesterol( LDL-C),lipoprotein( a) [Lp( a) ],homocysteine( Hcy) and high-sensitivity C reactive protein( hs-CRP) were determined in 83 elderly patients with hypertension. Ultrasonic inspection was performed,and the subjects were classified according to the status of Hp infection. The results were analyzed statistically. Results Hp infection occurred in 37 cases,and the Hp infection rate was 44. 6%. Carotid intima media thickness( CIMT) in Hp positive group was significantly higher than that in Hp negative group( P <0. 05). The serologic indices,LDL-C,Lp( a),Hcy and hs-CRP,in Hp positive group were significantly higher than those in Hp negative group( P < 0. 05). The serum levels of LDL-C,Hcy and hs-CRP in Hp current infection were significantly higher than those in Hp previous infection( P < 0. 05). Conclusions The Hp infection could be closely related with atherosclerosis,and Hp infection could increase the risk of carotid atherosclerosis in patients with hypertension.
