摘 要:目的 利用CT三维成像技术研究心房颤动(房颤)患者肺静脉结构的形态学特点。方法 采用GE Light Speed VCT,XT 64层螺旋CT对17例阵发性房颤患者与7例持续性房颤患者行心脏扫描,研究肺静脉口的最大径、最小径、截面积、圆度并进行对比分析。结果 右肺静脉开口3支变异达21%,左肺静脉开口3支变异达4%,右多支变异达4%;阵发性房颤组左下肺静脉最小径比持续性房颤组小,差异有统计学意义(P=0.029);而其他肺静脉口参数差异无明显统计学意义(P=0.082~0.968)。结论 持续性房颤患者的肺静脉口直径较阵发性房颤患者增宽。Objective To study the pulmonary vein morphology in atrial fibrillation (AF) patients by using CT threedimensional imaging technique. Methods 64-slice CT scanning was performed in 17 patients with paroxysmal atrial fibrillation and 7 patients with persistent atrial fibrillation. The maximum diameter, minimum diameter, cross section areas and roundness of the pulmonary vein were measured. The results were analyzed. Results Three branches variation of right pulmonary vein opening orifice was seen in 21% of patients, three branches variation of left pulmonary veins opening orifice in 4% of patients, and the right multiple branches in 4% of patients. Minimum diameter of left inferior pulmonary vein in the paroxysmal AF group was much smaller than that in persistent AF group, and the difference was statistically significant (P = 0.029 ), while the differences in other parameters of pulmonary vein ostium were not statistically significant ( P = 0.082 - 0.968). Conclusion The diameter of the pulmonary vein ostium in patients with persistent atrial fibrillation is wider than that in patients with paroxysmal atrial fibrillation.
