摘  要:目的探讨经岩下窦入路用Onyx胶结合弹簧圈栓塞海绵窦区硬脑膜动静脉瘘(CSDAVF)的应用价值。方法回顾性分析10例CSDAVF患者应用Onyx胶结合弹簧圈栓塞的技术特点并评价疗效。结果 8例治疗后即刻造影显示海绵窦及瘘口完全栓塞;2例海绵窦被栓塞,残留少量岩下窦引流,经压颈治疗后症状消失。术后3个月随访行全脑血管造影检查无变化,未予再处理。结论采用Onyx结合弹簧圈经岩下窦入路栓塞CSDAVF是安全有效的,对自发性CSDAVF可作为首选方法。Objective To investigate the therapeutic effect of Onyx and detachable coil embolization via inferior petrosal sinus access for cavernous sinus dural arteriovenous fistulas, and to discuss its technical points. Methods Embolization treatment with Onyx and detachable coil via inferior petrosal sinus access was successfully carried out in 10 patients with cavernous sinus dural arteriovenous fistulas. The clinical data were retrospectively analyzed, and the therapeutic resuits were evaluated. Results The procedure was successfully accomplished in all 10 patients. Angiography performed immediately after embolization showed that the fistula was completely obstructed in 8 cases; cavernous sinus was embolized in 2 cases leaving slight inferior petrosal sinus draining, which disappeared after oppression of the carotid artery. Whole cerebral angiography was performed three months after the operation, which showed that the vascular condition was stable and no further treatment was needed. Conclusion For the treatment of cavernous sinus dural arteriovenous fistulas, embolization treatment with Onyx and detachable coil via inferior petrosal sinus access is safe and effective. This technique should be recommended as the treatment of first choice for spontaneous cavernous sinus dural arteriovenous fistulas.
