摘  要:目的探讨急性脑梗死患者肾功能损害与脑白质疏松(WMLs)之间的联系。方法急性脑梗死患者114例,根据MRI检查结果分为WMLs组(67例)和非WMLs组(47例),分析血清胱抑素C(Cys-C)水平升高是否构成WMLs发生的独立预测因素;根据Cys-C异常与否分为Cys-C正常组和Cys-C升高组,分析Cys-C水平与WMLs严重程度之间的相关性。结果 WMLs组和非WMLs组之间在年龄、高血压和Cys-C水平上存在统计学差异(P<0.01或P<0.05);Cys-C升高是WMLs发生的独立预测因素(P<0.05);Cys-C升高组WMLs严重程度高于Cys-C正常组(P<0.05);Cys-C水平与WMLs严重程度存在正相关性(r=0.308,P<0.05)。结论急性脑梗死患者血清Cys-C水平升高能预测WMLs的发生,并评估WMLs的严重程度;该群体患者肾脏损害与WMLs之间存在正相关性。Objective To investigate the relationship between serum cystatin C(Cys-C)and cerebral white matter disease lesions(WMLs)in the patients with acute cerebral infarction.Methods According to MRI results,114 patients with acute cerebral infarction were divided into group WMLs(67cases)and group non-WMLs(47cases).The value of the increase of Cys-C in predicting the occurrence of WMLs was analyzed.On the basis of serum level of Cys-C,the patients were divided into groups of N(with normal Cys-C)and E(with elevated Cys-C),and the correlation of WMLs severity with serum level of Cys-C was analyzed.Results The Age,hypertension and Cys-C were statistically different between groups of WMLs and non-WMLs(P〈0.01 or P〈0.05).The increase of Cys-C was an independent predictor for appearance of WMLs(P〈0.05).The severity of WMLs was more in group E than that in group N(P〈0.05).Serum level of Cys-C was positively correlated with the severity of WMLs(r=0.308,P〈0.05).Conclusion The increase of serum Cys-C can predict the appearance of WMLs and evaluate the severity of WMLs in the patients with acute cerebral infarction.There is a positive correlation between renal damage and WMLs.
