摘 要:目的分析甲型H1N1流感(甲流)患儿血常规特点及C反应蛋白水平特点,为初步诊断儿童甲流提供帮助。方法选择205例甲流患儿、135例普通流感患儿和60名健康对照儿童,统计分析白细胞计数(WBC)、血小板计数(PLT)、平均血小板体积(MPV)、中性粒细胞百分比、淋巴细胞百分比、单核细胞百分比和全血CRP水平。三组间计量资料比较采用ANOVA单因素方差分析,两两组间计量资料比较采用SNK-q检验。结果甲流组、普通流感组和健康对照组WBC均值分别为(6.83±2.80)×109、(8.61±4.07)×109、(8.13±1.99)×109,PLT总数均值分别为(242.44±85.70)×109、(326.74±116.03)×109、(248.67±42.16)×109,MPV均值分别为(8.62±0.95)fl、(8.40±0.78)fl、(9.41±0.61)fl。三组间WBC差异无统计学意义(F=2.6,P>0.05),PLT总数差异有统计学意义(F=7.150,P<0.05),MPV差异有统计学意义(F=5.981,P<0.05)。甲流组患者淋巴细胞和中性粒细胞百分比与健康对照组比较差异无统计学意义(q值分别为0.57、2.06,P值均>0.05),但单核细胞百分比明显高于健康对照组和普通流感组,差异有统计学意义(q值分别为8.17、5.39,P值均<0.05)。结论甲型H1N1流感感染早期儿童血常规WBC无显著变化,单核细胞比例和CRP水平增高,而PLT总数低于普通流感患儿,可为鉴别普通流感和甲流提供参考。Objective To explore the characteristics of complete blood count in the children with H1N1 influenza and the levels of C-reactive protein. Methods 205 cases of children with H1N1 influenza, 135 with ordinary influenza and 60 healthy children as healthy controls were selected for the study. Statistical analysis was conducted on white blood cell count(WBC), platelet count(PLT), mean platelet volume(MPV), percentages of neutrophils, lymphocytes and monocytes,and levels of whole blood CRP. Data of the three groups were compared using ANOVA single-factor analysis of variance, and data between two groups were compared using SNK-q test. Results The arithmetic mean of WBC in the group with H1N1 influenza was(6.83±2.80)×10^9, the group with common influenza was(8.61±4.07)×10^9, and the healthy control was(8.13±1.99)×10^9.The arithmetic mean of PLT in group with H1N1 influenza was(242.44±85.70)×10^9, the group with common influenza was(326.74±116.03)×10^9, and the healthy control was(248.67±42.16)×10^9. The three groups showed no significant difference on the WBC(F=2.6, P0.05), but significance differences on the PLT number(F=7.150, P0.05) and MPV(F=5.981, P0.05).The percentage of lymphocytes and neutrophils of the children with H1N1 influenza showed no significant difference from healthy controls(q=0.57, 2.06, P0.05). But the percentage of monocytes was significantly higher than that of the healthy control and the common influenza group(q=8.17, 5.39,P0.05). Conclusion The WBC shows no significant change in the children with H1N1 influenza, with a increased proportion of monocyte and elevated CRP level. The PLT of the group with H1N1 influenza is less than that of the common influenza group, which may help us to detect and discriminate H1N1 influenza.
