桡动脉充气止血绑带在冠状动脉介入术中的效果评价

摘　　要：目的探讨桡动脉充气止血绑带在桡动脉途径经皮冠状动脉介入术(PCI)后的有效性及安全性。方法选取2012年于山东大学附属省立医院接受经皮冠状动脉介入术患者共1069例。随机分为两组,实验组使用桡动脉充气止血绑带压迫止血,对照组使用常规的绷带压迫止血。观察比较两组止血时间、制动时间、患者压迫点局部情况、术后舒适度及并发症发生率。结果两组患者年龄、性别、基础疾病等方面比较差别无统计学意义(P>0.05)。实验组止血时间和制动时间均短于对照组,差异有统计学意义(P<0.05),并发症发生率少于对照组(P<0.05)手部肿胀程度(轻度、中度、重度)发生率均少于对照组(P<0.05),两组患者满意度比较有统计学差异(P<0.05)。结论经桡动脉途径PCI术后应用桡动脉充气止血绑带安全有效,止血时间及制动时间短,并发症少,同时给予专业护理干预,提高了患者舒适度与满意度,值得临床推广。Objective To research the radial pneumatic tourniquet application and nursing in percutaneous coronary intervention and to analyze the effectiveness and safety of this therapeutic measure. Methods We retrospectively analyzed the clinical data of 1069 cases who underwent pereutaneous coronary inter- vention through radial artery in our hospital in 2012. They were divided into two groups according to the method of pneumatic tourniquet application. There were 535 patients in observation group who were treated with radial pneumatic tourniquet and 534 cases in control group who were treated with the common bandage. Time for hemostasis and local immobilization, status of the compre-ssing area, comfort level after operations and incidence of complications of both groups were observed. Results There were no significant differences in age, gender and underlying diseases between two groups （ P 〉0.05）. The time for hemostasis and immoblization were both shorter in the observation group than in the control group significantly （P〈0.05）. The status of compressing area and the comfort level were significantly better in the observation group, and the complication rate was lower （ P 〈0.05）. Conclusion The radial pneumatic tourniquet was safe and effective after the operation of percutaneous coronary intervention. With shorter time of hemostasis and local immo-bilization, less complications, radial pneumatic tourniquet is more comfortable, satisfying and is worth popularizing .
