摘  要:目的初步探讨声脉冲辐射力成像(ARFI)在评估糖尿病患者肾脏弹性硬度中的价值。方法选取单纯糖尿病组19例,糖尿病肾病组18例,体检健康者38例为对照组,ARFI检测其肾皮质、肾髓质及肾窦部的弹性指数(EI)。结果对照组、单纯糖尿病组及糖尿病肾病组的EI均在肾皮质、肾髓质、肾窦依次减小(P<0.01)。肾皮质、肾髓质和肾窦部EI在对照组、单纯糖尿病组及糖尿病肾病组间依次减小(P<0.05)。结论 ARFI能无创地反映肾脏组织不同部位的弹性程度,早期发现糖尿病肾脏组织的弹性异常,对临床评估糖尿病肾脏损害具有一定的价值。Objective To investigate the value of acoustic radiation force impulse(ARFI)technology in evaluating the elastic stiffness of renal tissue in diabetics. Methods Thirty-eight healthy adults were used as the control group,19 diabetics without diabetic nephropathy and 18 patients with diabetic nephropathy were included. All patients underwent ARFI examination. The elastic indexes(EI)were measured in the renal cortex,medulla and sinu. Results The EI decreased successively in renal cortex,medulla and sinu in all patients. In healthy adults,patients without diabetic nephropathy and patients with diabetic nephropathy,the EI in renal cortex,medulla and sinu decreased progressively. Conclusion ARFI technique can noninvasively reflect the elastic stiffness in the different parts of kidney,early detect the changes of renal elastic compliance in the diabetics, which has a certain value in assessing the renal damage.
