益心舒胶囊联合低分子肝素钙治疗慢性肺源性心脏病急性加重期患者的临床观察

摘　　要：目的:观察益心舒胶囊联合低分子肝素钙注射液治疗慢性肺源性心脏病(简称肺心病)急性加重期患者的疗效。方法:选取我院2012年1月至2014年2月的住院患者80例,分为两组(各40例)。对照组给予常规吸氧、平喘、化痰、扩张支气管、合理抗生素及减轻心脏负荷等综合治疗;治疗组在对照组治疗基础上加用益心舒胶囊(每次3粒,每日3次,疗程28 d)与低分子肝素钙注射液(1 ml/d,皮下注射,疗程14 d)。结果:治疗组有效率显著高于对照组(P<0.05)。血气分析指标方面,治疗组PaO2、PaCO2的改善均明显优于对照组(P<0.05)。血液流变学方面,与治疗前相比,治疗组治疗后的全血高切黏度、全血低切黏度、血浆黏度、红细胞比容、纤维蛋白原、血红蛋白定量和红细胞计数等指标均明显下降(P<0.05);与对照组治疗后比较,治疗组各项指标均明显下降(P<0.05)。结论:益心舒胶囊联合低分子肝素钙注射液治疗慢性肺心脏病急性加重期患者安全、有效,无明显副作用。OBJECTIVE：To evaluate the efficacy of Yixinshu capsules plus Low molecular weight heparin calcium injection for acute exacerbation of chronic pulmonary heart disease. METHODS：Eighty patients hospitalized in our hospital between Jan.2012 and Feb. 2014 were assigned to two groups of 40 each. The control group was routinely given comprehensive treatment including oxygen inhalation,antiasthmatic and phlegm-relieving drugs,bronchodilators,antibiotics and drugs for reducing cardiac stress,while the treatment group was given add-on Yixinshu capsules（3 capsules each time,3 times daily）for 28 days plus Low molecular weight heparin calcium injection（1 ml daily）subcutaneously for 14 days. RESULTS：The effective rate in the treatment group was significantly higher than in the control group（P0.05）. Blood gas analysis showed significantly better improvement in pH,PaO2 and PaCO2in the treatment group than in the control group（P0.05）. With regard to hemorheology,the indicators including high shear whole blood viscosity,low shear whole blood viscosity,plasma viscosity,hematocrit,fibrinogen,hemoglobin quantitation and red blood cell count were all significantly decreased in the treatment group after treatment as compared with before treatment（P0.05）;compared with the control group,indicators in the treatment group were significantly decreased after treatment（P0.05）. CONCLUSIONS：Treatment for acute exacerbation of chronic pulmonary heart disease with Yixinshu capsules plus Low molecular weight heparin calcium injection is proved to be safe and effective.
