通莲Ⅰ号方联合适形放疗对中晚期食管癌患者免疫功能的保护作用

摘　　要：目的观察通莲Ⅰ号方联合适形放疗对中晚期食管癌患者血清TNF-α、IL-2R和CP的影响，揭示该方对患者免疫功能的调节机制。方法35例健康人作为对照，38例中晚期食管癌患者，年龄45～65岁，患者均采用三维适形放疗，总剂量（56—66）Gy／（25—30）次／（5—6）周，配合通莲Ⅰ号方1剂／d，2次／d。结果治疗前，患者血清TNF—α、IL-2R和CP含量和正常人比较，有统计学意义（P〈0．01）。治疗后和正常人比较，3项指标均无统计学意义（P〉0．05）。治疗后6个月复发患者13例，未复发18例。复发患者IL-2R极显著升高，与未复发组比较，有统计学意义（P〈0．01）；TNF-α和CP也显著升高，与未复发组比较，有显著性差异（P〈0．05）。结论通莲Ⅰ号方联合适形放疗可以显著改善患者短期免疫功能，长期疗效有待观察。Objective To observe effect of Tonglian Decoction（TD） combined with three-dimensional conformal radiotherapy （3DCRT） on serum TNF-α, IL-2R and CP of patients with advanced esophageal cancer and reveal its regulatory mechanism on immunologic function of patients.Methods 38 patients （45-65 years） with advanced esophageal cancer were selected and controlled with 35 healthy volunteers. Patients were treated by three -dimensional conformal radiotherapy （Total dose （56-66） Gy/（25-30） times/（5-6） weeks combined with Tonglian Decoction：one dose a day, two times a day）. Results The contents of TNF-α, IL-2R and CP of patients before treatment had significant difference compared with those of healthy volunteers （ P 〈 0.01 ）. Three factors shows no difference （ P 〉 0.05） compared with healthy volunteers after treatment. Howerver, there were 13 cases of relapse patients and 18 cases of non-recurrence in 6 months after treatment. IL-2R of relapse patients increased dramatically with significant differences （P 〈 0.01）. TNF-α and CP of patients increased dramatically too. There are significant differences （P 〈 0.05）. Conclusion TD combined with 3DCRT in the treatment of advanced esophageal cancer can significandy improve the immunologic function of patients in short-term stage. Its long-term efficacy remains to be seen.
