辅助生殖技术中HBV携带者胚胎发育和IVF结局的回顾分析

摘　　要：【目的】探讨乙肝病毒(HBV)感染以及不同感染模式的夫妇胚胎发育情况和体外授精/卵胞浆内单精子显微注射(IVF/ICSI)结局。【方法】回顾性分析行IVF/ICSI治疗,637个HBV感染患者周期与1 892个未感染者周期的基础参数以及临床结局,同时进一步比较不同模式HBV感染者之间的临床结局。基础参数包括女方年龄、月经第3天基础内分泌水平、不孕原因、原发还是继发不孕、不孕年限、第几次周期、卵巢刺激方案是长方案还是短方案、IVF还是ICSI。临床结局包括比较各组人群在获卵数、受精率、卵裂率、着床率、临床妊娠率、流产率方面有无差异。【结果】一方或双方感染HBV的夫妻与未感染的夫妻在基础参数方面没有显著性差异。获卵数、受精数、卵裂数、优质胚胎数、冷冻胚胎数、着床率、妊娠率以及流产率与未感染组比较没有差异(P均>0.05),不同模式的HBV感染者之间着床率、妊娠率和早期流产率均没有显著差异(P均>0.05)。【结论】HBV感染夫妇的胚胎继续发育的潜能和IVF结局与未感染者相似。不同模式的HBV感染者IVF结局相似。[Objective]To evaluate embryo development and IVF/ICSI outcome of HBV infected couples compared with uninfected couples and different serological profile types with HBV.[Methods]Analysis of the characters and outcomes of 637 HBV infected couples compared with 1892 uninfected couples and the outcomes of different serological profile types with HBV after IVF or ICSI treatment was performed retrospectively.Characters were including：maternal age,the time of the procedure,serum FSH at day 3,LH,E2,PRL,T,etiology of the infertility,primary or secondary infertility,duration of the infertility,number of previous cycles,type of pituitary inhibition,IVF procedure.Embryo development criteria such as number of oocytes,fertilization rate,cleavage rate,number of good embryos and used for vitrification,implantation rate,clinical pregnancy rate and miscarriage rate were compared between the two groups or subgroups.[Results]There were no differences between the two groups of patients for the characters.There were no difference in embryo development parameters,implantation rate,clinical pregnancy rate and miscarriage rate between the two groups （P 〉 0.05）.There were no differences in implantation rate,clinical pregnancy rate and abortion rate among HBV infected subgroups （P 〉 0.05）.[Conclusion]The embryo development and IVF outcome of HBV infected couples seeking for assisted reproduction treatment were similar to the uninfected couples.HBV infected patients with different serological profile types had the similar outcome.
