负压封闭引流联合水胶敷料对深部感染创面患者临床预后分析

摘　　要：目的探讨分析水胶体敷料联合负压封闭引流(VSD)对深部感染创面患者的临床预后,总结临床经验,为今后深部感染创面患者的治疗提供理论依据。方法选取医院2013年6月-2014年6月收治的80例深部感染患者,将其随机分为两组,各40例,对照组行VSD联合普通无菌纱布治疗,治疗组行水胶体敷料联合VSD治疗,比较两组患者换药时的VAS疼痛评分、治疗后的细菌清除率以及清洁创面所需时间等指标。结果患者换药时VAS评分治疗组为(2.28±1.13)分、对照组为(4.39±1.61)分,两组比较差异有统计学意义(P<0.05);患者的细菌清除率治疗组为92.5%、对照组为72.5%,两组比较差异有统计学意义(P<0.05);治疗组的堵管率、平均创面清洁时间分别为0、11d、对照组分别为47.5%、26d,两组比较差异有统计学意义(P<0.05)。结论水胶体敷料联合VSD治疗深部感染创面患者的预后良好,可缩短创面清洁时间,减少患者住院时间,提高细菌清除率,防止引流管堵漏,减少更换引流管的次数,缓解患者痛苦。OBJECTIVE To explore the clinical prognosis of the patients with deep wound infections who were trea-ted with hydrocolloid dressings combined with vacuum sealing drainage （VSD） and summarize the clinical experi-ence so as to provide theoretical basis for treatment of the patients with deep wound infections .METHODS A total of 80 patients with deep infections who were treated in the hospital from Jun 2013 to Jun 2014 were enrolled in the study and randomly divided into the two groups ,with 40 cases in each ,then the control group was treated with VSD combined with common sterile gauze ,while the treatment group was given the hydrocolloid dressings com-bined with VSD ;the VAS score at dressing ,bacterial clearance rate after the treatment ,and time of debridement were observed and compared between the two groups .RESULTS The VAS score at dressing was （2 .28 ± 1 .13） points in the treatment group ,（4 .39 ± 1 .61） points in the control group ,and there was significant difference between the two groups （P〈0 .05）;the bacterial clearance rate was 92 .5% in the treatment group ,72 .5% in the control group ,and there was significant difference between the two groups （P〈0 .05）;the plugging rate was 0 in the treatment group , 47 .5% in the control group ;the average wound debridement time was 11 days in the treatment group ,26 days in the control group ,and there were significant differences between the two groups （P〈0 .05） .CONCLUSION The hydrocol-loid dressings combined with VSD can achieve good prognosis for the patients with deep wound infections ,shorten the wound debridement time ,reduce the length of hospital stay ,raise the bacterial clearance rate ,prevent the drainage tube from plugging ,reduce the frequency of change of drainage tube ,and alleviate the sufferings of the patients .
