成都中心城区及周边农村儿童哮喘治疗情况的对比分析

摘　　要：目的：对比分析成都中心城区及周边农村0～14岁城乡哮喘患儿的治疗情况。方法采取随机、整群和不等比抽样方法抽出成都市中心城区（青羊区）、附近农村（金堂县竹篙镇）所需调查的学校、幼儿园及社区的全部0～14岁儿童，向家长发放问卷初筛表，筛选出哮喘的可疑患者，经呼吸专科医师问诊和体查以明确诊断。采用Epi Info 建立数据库，进行调查问卷的录入及逻辑筛查。结果成都市中心城区的哮喘患儿累计患病率高于农村，患儿哮喘发作减轻或完全缓解情况高于农村，接受规范化治疗、使用指南推荐的疗效佳且副作用少的药物使用率明显高于农村地区，差异有统计学意义（P ＜0.05）。结论不同生活环境、医疗卫生资源和经济收入，哮喘的治疗和疾病转归存在差异，今后防治工作的重点应注重广大基层地区，增加卫生资源的投入、加强对医务人员及患者普及推广哮喘基本防治知识的宣传和教育。Objective To compare the treatment status of children with asthma lived under conditions of different living envi -ronment ,economic level and sanitation in Chengdu area .Methods All children aged 0-14 years old from investigated schools ,kinder-gartens and communities in urban （ Qingyang district ） and rural area （ Zhugaozhen of Jintang County ） were involved in the study by u-sing a random,cluster and disproportionate sampling method .First at all,questionnaire screening tables were filled by parents ,and then suspicious asthmatic patients were diagnosed by respiratory specialists .Data base was created by Epi Info with recordings and logistic screening .Results The cumulative prevalence of children with asthma in urban area was higher than that in rural area .Reduced or completely remission of asthma in urban children was higher than that in rural area .Compared to rural children ,frequency of standard-ized treatment and usage of effective drugs with little adverse reactions recommend by guidelines was more in urban children .All differ-ences were statistically significant （ P 〈0.05 ） .Conclusion The treatments and outcomes of asthma were influenced by living envi-ronment,economic level and sanitation .Therefore,more prevention and control work should be paid on rural area by increasing health resources,and strengthening propaganda and education of basic knowledge about prevention and cure of asthma to both medical staffs and patients.
