针药结合治疗肾虚痰湿型多囊卵巢综合征40例远期疗效观察

摘　　要：目的:观察补肾化痰中药联合针刺对肾虚痰湿型多囊卵巢综合征(PCOS)的远期临床疗效及其对性激素、瘦素、胰岛素抵抗的影响。方法:将80例体重指数(BMI)≥25 kg/m2的肾虚痰湿型PCOS患者随机分为治疗组40例,给予补肾化痰中药联合针刺治疗;对照组40例,给达英-35治疗。两组均治疗3个月后停药3个月,观察其临床疗效:月经及妊娠情况、体重、黄体生成素(LH)/卵泡刺激素(FSH)、睾酮(T)、瘦素(Leptin)、血糖(FPG)、胰岛素(FINS)的变化,评估胰岛素抵抗(IR)。结果:治疗组总有效率70.0%;对照组总有效率52.5%。两组T、BMI、Leptin、FINS、IR治疗停药后3个月较治疗前均降低(P<0.05),治疗组LH、LH/FSH治疗停药后3个月较治疗前降低(P<0.05);治疗停药3个月后,治疗组体重较对照组体重降低(P<0.05),治疗组LH、LH/FSH、T、BMI、Leptin、FINS、IR低于对照组(P<0.05),均有统计学意义。结论:补肾化痰中药加针刺结合治疗体重指数较高的肾虚痰湿型PCOS远期临床疗效较达英-35效果显著,可有效降低体重、雄激素、瘦素、胰岛素抵抗水平,可指导临床治疗多囊卵巢综合征。Objective：To observe the long -term clinical curative effect of TCM and acupuncture on polycystic ovary syndrome （PCOS） and its influence on sex hormones,leptin and insulin resistance. Methods ：80 cases whose BMI was greater than 25 were randomly divided into treatment group （40 cases, tonifying kidney and resolving phlegm and activating blood formula combined with acupuncture method treatment） and the control group （ 40 cases, Diane - 35 ）. 3 months after treatment and stopped three months, its clinical curative effect was observed. Menstruation and pregnancy, weight, luteinizing hormone （LH） and follicle - stimulating hormone （ FSH ） and testosterone （T）, Leptin, Leptin, blood glucose （ FPG）, insulin （FINS）, insulin resistance （IR） were assessed. Results ：The total effective rate was 70.0%. The control group＇s total effective rate was 52.5%. The weight loss in treatment group was less than that of the control group（ P 〈 0.05 ）. Two groups＇ T, BMI, Leptin ,FINS and IR after discontinuation of treatment for 3 months reduced significantly（ P 〈 0.05 ）. The treatment group＇s LH and LH/FSH after discontinuation of treat- ment for 3 months reduced significantly （ P 〈 0.05 ）. After treatment, treatment group＇s LH, LH/FSH,T, BMI, Leptin, FINS and IR were lower than those of the control group （ P 〈 0.05 ）. Conclusion ： The combination of TCM formula and acupuncture of PCOS and higher body mass index has the same the long - term clinical curative effect as Diane - 35 and can effectively reduce the body weight, androgen levels, leptin and insulin resistance, guiding the clinical treatment of polycystie ovary syndrome.
