摘　　要：目的探讨不同麻醉方式对老年腹腔镜结直肠手术患者的影响。方法 100例腹腔镜下结肠癌或直肠癌根治术患者随机分为吸入全身麻醉组(I组)和硬膜外复合吸入全身麻醉组(C组),每组50例。分别于术前、术后30 min、术后3 h采用运动活动评分(MAAS)评估镇静程度,并在相应时间点采静脉血样检测血糖(Glu)、皮质醇(Cor)、去甲肾上腺素(NE)、肾上腺素(E)。结果 C组患者术后30 min MAAS评分明显低于I组(P<0.05),I组术后30 min、术后3 h Cor、Glu、NE、E明显高于术前及C组(P<0.05)。结论硬膜外复合全身麻醉有助于提高患者苏醒质量并减轻应激反应,适用于老年患者行腹腔镜结直肠手术。Objective To investigate the influence of different anesthesia methods on elderly patients undergoing laparoscopic colorectal surgery. Methods A total of 100 cases of laparoscopic resection of colon or colorectal cancer were randomly divided into the inhalation general anesthesia group （group I） and epidural composite general anesthesia group （Group C）, and each group contained 50 cases. Motion activity assessment scale （MAAS） was applied to assess the degree of sedation, blood glucose （Glu）, cortisol （Cot）, norepinephrine （NE） and epinephrine （E） before surgery and 30 rain and 3 h after surgery. Results MAAS score of group C was obviously lower than that of group I （P〈0.05）. The levels of Glu, Cot, NE and E of group I after 30 rain and 3 h of surgery were all higher than those score before the surgery and those of group C （P〈0.05）, Conclusion Epidural composite general anesthesia can improve awaking quality and reduce stress reaction, and is suitable for laparoscopic colorectal surgery of elderly patients.
