N末端B型钠尿肽前体与老年心功能不全相关性的临床研究

摘　　要：目的探讨N末端B型钠尿肽前体(NT-proBNP)与老年心功能不全的相关性。方法将收集的老年心功能不全患者100例作为试验组,选取同期入院接受心脏检查结果正常的同年龄段的患者80例作为对照组,采用免疫放射法测定NT-proBNP水平,动态观察NT-proBNP水平与老年心功能不全患者心功能变化及预后的相关性。结果试验组患者NT-proBNP水平、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、心肌做功指数(Tei指数)明显高于对照组,而左心室射血分数(LVEF)、左心室短轴缩短率(LVFS)、E/A比值明显低于对照组,两组间所有参考指标比较差异均有显著性(P均<0.05);试验组患者NT-proBNP水平、LVEDD、LVESD、Tei指数随心功能级别的升高逐渐升高,LVEF、LVFS、E/A比值随心功能级别降低逐渐降低,经方差分析,差异具有显著性(P<0.05);LVEDD、LVESD、Tei指数与NT-proBNP水平呈正相关,影响大小顺序为LVESD、LVEDD、Tei指数。LVEF、LVFS以及E/A比值与NT-proBNP水平呈负相关,影响大小顺序为E/A比值、LVEF、LVFS。结论 NT-proBNP与老年心功能不全各项指标密切相关,心功能不全级数越高,NT-proBNP水平越高。Objective Study of N terminal B type natriuretic peptide precursor （NT-proBNP） in elderly patients with heart failure correlation. Method Collected 100 old heart function insufficiency patients as test group, selected the same hospital cardiac examination and the results of normal section of the same age in 80 cases as control group, NT-proBNP levels were measured by RIA, correlation between the change of heart function and prognosis of dynamic observation of NT-proBNP in elderly patients with heart failure patients. Result The test group NT-proBNP, left ventricular end diastolic diameter （LVEDD）, left ventricular end systolic diameter （LVESD）, myocardial performance index （Tei index） were significantly higher than that in control group, and the left ventricular ejection fraction （LVEF）, left ventricular fractional shortening （LvFS） and E/A ratio were significantly lower than those in the control group. Between the two groups were all reference index issignificant difference （P 〈 0.05）; elevated NT-proBNP level in test group, LVEDD, LVESD, Tei index of heart function level gradually increased, LVEF, LVFS, E/A ratio decreased heart fimction level decreased gradually, through analysis of variance between levels of comparative difference was significant （P 〈 0.05）; there was positive correlation between LVEDD, LVESD, Tei index and NT-proBNP level effects in the order of LVESD, LVEDD, Tei index. LVEF, LVFS and E/A ratio was negatively correlated with NT-proBNP level, factor in the size of the order E/A ratio, LVEF, LVFS. Conclusion Closely related to N end B precursor m elderly patients with cardiac function of natriuretic peptides not all indicators, heart function not entire series is higher, the higher the NT-proBNP level.
