半月板成型缝合术在盘状半月板撕裂中的近期临床效果评价

摘　　要：目的评价半月板成形术结合Fastfix及MM-Ⅱ缝合技术在膝关节外侧盘状半月板撕裂中的近期临床效果。方法 2010年1月至2012年12月,对59例膝关节盘状半月板撕裂患者进行了半月板成型及缝合术。根据撕裂的大小和部位,采用Fast-fix缝合系统及MM-Ⅱ缝合套管针进行缝合。59例随访12~26个月,平均（18.4±3.6）个月。在术前及术后随访时记录患者的膝关节活动度、稳定性及Lysholm评分,比较术前及末次随访的Lysholm评分来评价盘状半月板成型缝合术的近期临床疗效。结果所有病例在末次随访时没有关节交锁及失稳症状,关节活动度均恢复正常。Lysholm评分从术前（58.8±6.8）分提高至术后（93.3±2.4）分,经t检验差异有统计学意义（t=-38.24,P〈0.01）。结论对于膝关节盘状半月板合并撕裂,在进行成型手术后通过Fast-fix及MM-Ⅱ对半月板撕裂进行缝合修复,可获得满意近期疗效。Objective To observe the short -term effects of the meniscus suture with Fastfix in the repair of the torn discoid meniscus .Methods 59 cases of lateral discoid meniscus tear were operated in Union hospital in 2010.1 ~2012.12.The age was from six to 38 years old.There were 37 cases of complete discoid meniscus and 22 cases of incomplete discoid meniscus .The longitudinal tears involved the popliteal hiatus was found in 13 cases （ three cases of bucket tear ） .38 cases were longitudinal and lamellar tear （ eight cases of bucket tear ） , while the anterior horn tear was found in eight cases .All the patients were operated with arthroscope; the meniscoplasty was carried out firstly , then the tear was sutured with Fast-fix and MM-Ⅱ.The lower limb was fixed with brace after the operation;total weight bearing was avoided until eight weeks after the operation , while the early flexion exercises were carried out , and less than 90 degrees flexion was achieved within four weeks after the surgery .59 cases were followed up for （18.4 ±3.6） months （range from 12-26 month）.The range of motion, stability, joint function, and Lysholm scores were recorded pre and post -operatively.Results In all the cases, the mobility of the knee joint recovered to normal level with no recurrence of locking or instability .Lysholm scores were improved from （58.8 ±6.8） preoperatively to （93.3 ±2.4） postoperatively.Comparing the data with the paired T test, the difference of Lysholm scores between the pre-operation and postoperation was statistically significant （t=-38.24, P〈0.01）.Conclusion The early clinical outcomes of meniscoplasty and suture were satisfactory in torn discoid lateral meniscus;the Fast-fix and MM-Ⅱsuture technique is simple and efficient .
