摘  要:目的调查耐甲氧西林金黄色葡萄球菌(MRSA)6种耐药基因与4种毒力基因的携带状况,分析其阳性率及阳性模式。方法收集2011年1-12月江苏省江阴市及浙江省宁波市2所三级医院住院患者临床标本中分离到的31株金黄色葡萄球菌,用聚合酶链反应(PCR)法分析6种耐药基因(mecA、aac(6′)/aph(2″)、aph(3′)-Ⅲ、ermA/B/C、tetM、qacA/B)与4种毒力基因(sasX、psm-mec、pvl、tst)。结果 31株金黄色葡萄球菌mecA、aac(6′)/aph(2″)、aph(3′)-Ⅲ、ermA/B/C、tetM、qacA/B、sasX、psm-mec、pvl、tst阳性率分别为100.0%、96.8%、77.4%、96.8%、90.3%、38.7%、74.2%、93.5%、96.8%、0,可分为7种阳性结果模式。结论对MRSA进行6种耐药基因及4种毒力基因检测研究尚为国内外首次;MRSA携带耐药基因和毒力基因,与人体感染性疾病的种类及与病程、转归有关,值得作多中心大规模调查。OBJECTIVE To investigate the distribution of 6 drug resistance genes and 4 virulence genes in a group of 31 methieillin-resistant Staphylococcus aureus (MRSA) isolates and analyze the positive rates and positive modes. METHODS From Jan 2011 to Dec 2011, totally 31 strains of S. aureus were isolated from clinical specimens obtained from the hosptialized patients from two three-A hospitals in Jiangyin of Jiangsu province and Ningbo of Zhe-jiang province, then the 6 drug resistance genes, including mecA, aac(6′) /aph(2″), aph(3′)-Ⅲ, ermA/B/C, terM, and qacA/B as well as the 4 virulence genes, including sasX, psm-mec, pvl, andtst, were analyzed by using polymerase-chain-reaction (PCR) method. RESULTS Among the 31 strains of S. aureus, the positive rates of mecA , aac ( 6′) / a ph ( 2″) , a ph ( 31) -Ⅲ, ermA / B / C , tetM , qacA / B , sasX , psm-mec , pvl , and tst were 100.0%, 96.8%, 77.4%, 96.8%, 90.3%, 38.7%, 74.2%, 93.5%, 96.8%, and 0,respeetively; there were 7 positive modes. CONCLUSION It is for the first time to detect the 6 drug resistance genes and the 4 virulenece genes in MRSA in China. The MRSA' s carrying with the drug resistance genes and virulence genes is associated with the types of infectious disease and disease course as well as prognosis, and large scale of survery is worthy to be conducted.
