摘  要:目的研究免疫性血小板减少症(ITP)骨髓淋巴细胞共刺激分子表达与血小板抗体及临床特征的相关性,探讨ITP可能的发病机制。方法应用流式细胞技术检测ITP组68例和对照组20例骨髓淋巴细胞共刺激分子CD28+、CD80+和CD86+,酶联免疫吸附法检测患者血小板相关抗体(PAIgG);分析ITP患者表达异常的共刺激分子与其PAIgG、发病时间、就诊时血小板数值、治疗起效时间以及骨髓巨核细胞数值的相关性。结果 ITP组骨髓淋巴细胞CD80+和CD86+表达显著增高(P<0.01),CD28+无异常。ITP组79.41%患者PAIgG水平升高。ITP组CD86+表达率与骨髓巨核细胞数正相关(P<0.05),与就诊时血小板数负相关(P<0.05)。结论骨髓淋巴细胞CD80+、CD86+表达增高可能参与ITP免疫致病机制,CD86+表达与病变程度有关。[ Objective ] To investigate the expression and significance of costimulatory molecules in bone marrow lymphocytes of patients with immune thrombocytopenia (ITP). [Methods ] The eostimulatory molecules in the bone marrow lymphocytes were detected by flow cytometry in 68 ITP patients and 20 control subjects. The PAIgG of ITP patients were analyzed by ELISA method. [Results] The expression of eostimulatory molecules of CD28+ in bone marrow lymphocytes was normal in ITP. The expression of costimulatory molecule of CD80+ and CD86+ were higher in ITP than in controls (P 〈0.01). The PAIgG level (198.91±111,09) ng/107 PA was higher in 54 ITP cases. A posi- tive correlation was found between the expression of CD86+ and the number of megakaryocyte (P 〈0.05). A negative correlation was found between the expression of CD86+ and the platelet number(P 〈0.05). [ Conclusion ] The expres- sion of CD80+ and CD86+ in bone marrow lymphocytes may be related to the pathogenesis of ITP. The expression of CD86+ may correlate with severity of the ITP patiertts.
