连续硬膜外阻滞联合鼠神经生长因子注射治疗带状疱疹后遗神经痛90例临床观察

摘　　要：目的观察连续硬膜外阻滞内联合鼠神经生长因子注射带状疱疹后遗神经痛的临床疗效。方法按就诊时间先后顺序将90例持续疼痛1个月以上的带状疱疹后遗神经痛患者,分为Ⅰ组(n=30)为连续硬膜外神经阻滞联合鼠神经生长因子注射治疗组,Ⅱ组(n=30)为连续硬膜外阻滞治疗组,Ⅲ组(n=30)为鼠神经生长因子治疗组疗程3周。分别于治疗前,治疗后1、2和3周进行视觉模拟评分(VAS),并对患者的疗效进行评价。结果治疗3周后,连续硬膜外阻滞内联合鼠神经生长因子注射治疗组患者疼痛治疗总有效率96.67%,连续硬膜外阻滞治疗组疼痛治疗总有效率61.00%,鼠神经生长因子治疗组总有效率78.00%。结论连续硬膜外阻滞联合鼠神经生长因子注射治疗带状疱疹后遗神经痛起效迅速且效果显著,优于连续硬膜外阻滞治疗组和鼠神经生长因子治疗组。[ Objective ] To observe the clinical curative effect ofcontinuous epidural block combined with nerve growth factor injection on herpes zoster neuralgia. [Methods] According to the visiting time sequence of 90 patients with persistent pain for more than 1 months, postherpetic neuralgia patients were divided into group Ⅰ （n =30） for continuous epidural nerve block combined with nerve growth factor injection group, group Ⅱ （n =30） for continuous epidural block in the treatment group, group Ⅲ （n =30） rat nerve growth factor group for 3 weeks. Before and after treatment respectively, l, 2, 3 weeks of visual analogue scale （VAS） evaluation, and the curative effect evaluation. [ Results ] After 3 weeks treatment, continuous epidural block combined with nerve growth factor injection for pa- tients in pain treatment group was of total effective rate 96.67%, continuous epidural blockfor treatment of pain in treatment group was total effective rateof 61.00%, mouse nerve growth factor treatment group had total effective rate of 78.00%. [Conclusion] Continuous epidural block combined with nerve growth factor injection in treatment of neuralgia after herpes zoster has a rapid and significant effect, which is superior to continuous epidural block in the treatment group and the mouse nerve growth factor treatment group.
