胫骨远端骨折患者的手术治疗方案研究

摘　　要：目的探讨微创经皮钢板接骨术（MIPPO）与锁定加压钢板（LCP）技术结合治疗胫骨远端骨折的疗效。方法将120例胫骨远端骨折患者按照随机数字表法分为观察组（60例,给予MIPPO与LCP结合治疗）和对照组（60例,给予传统切开复位解剖型钢板内固定治疗）。比较2组患者的疗效。结果 2组各1例患者失访。2组患者随访时间为6~36个月,平均随访时间（19.4±6.5）个月。观察组患者的骨折愈合时间（5.4±0.8）个月显著短于对照组（7.8±1.5）个月,t=9.882,P〈0.05。观察组无并发症发生,对照组术后有3例切口部分坏死,1例患者发生骨髓炎。观察组中,24例优,32例良,3例可;对照组中,15例优,28例良,13例可,3例差。观察组的优良率（94.9%,56/59）显著高于对照组（72.9%,43/59）,χ2=10.602,P〈0.05。结论比起传统切开复位解剖型钢板内固定技术,MIPPO技术与LCP结合治疗胫骨远端骨折,能够明显缩短患肢骨折的愈合时间,临床疗效好。[ Objective ] To probe the effect of minimally invasive pereutaneous plate osteosynthesis （MIPPO） com- bined with locking compression plate （LCP） treatment on patients with distal tibial fracture. [Methods] 120 cases were randomly divided into observation group （60 cases, MIPPO combined with LCP treatment） and control group （60 cases, traditional open reduction and internal fixation with anatomic plate treatment）. The efficacy was analyzed. [ Results ] 1 case in each group was lost to follow-up. The duration of follow-up was 6-36 months in the two groups, and the average of follow-up times was （19.4±6.5） months. The time of fracture healing in observation group （5.4± 0.8） months was significantly shorter than that of control group （7.8±1.5） months, t =9.882, P 〈0.05. No complica- tions occurred in3 cases with partial necrosis of incision and 1 case with osteomyelitis in control group. There were 24 cases got excellent result, 32 case good, 3 case fair in observation group, while 15 cases got excellent result, 28 case good, 13 case fair, and 3 cases poor in control group. The excellent and good rate of observation group （94.9%, 56/59） was significantly higher than that of control group （72.9%, 43/59）, X2=10.602, P 〈0.05. [ Conclusions ] The treatment of MIPPO combined with LCP could significantly shorten the time of fracture healing, and has a good curative effect compared with traditional open reduction and internal fixation with anatomic plate treatment.
