兔肝VX2肿瘤射频消融术后增强CT表现及病理特征

摘　　要：目的评价射频消融(RFA)后兔肝VX2肿瘤炎性带及局部肿瘤进展(LTP)的增强CT表现及病理特征。方法建立兔肝VX2肿瘤RFA动物模型,于RFA后1(A组)、3(B组)、7(C组)、14天(D组)行增强CT及病理检查,观察不同时期炎性带及LTP的CT表现及病理特征。结果炎性带强化于RFA后第3、7天达到高峰,第14天明显减低;动脉期LTP表现为消融灶旁新月状、结节状异常强化影;延迟期D组炎性带CT值较LTP减低(P<0.05)。结论兔肝VX2肿瘤RFA术后增强CT表现与病理表现一致,能准确反映RFA术后不同时期炎性带和LTP的血供特点及变化;RFA术后早期炎性带与LTP的增强CT表现存在重叠,RFA术后CT增强随访宜在炎症消退期。Objective To assess the enhanced CT manifestations and pathology features of inflammatory and local tumorprogress （LTP） in VX2 liver tumor of rabbits after radiofrequancy ablation （RFA）. Methods In 28 rabbits, hepatic VX2tumor was developed and underwent RFA treatment. Pathological evaluation and enhanced CT scan were performed on the1st （group A）, 3th （group B）, 7th （group C） and 14th day （group D） after RFA. The imaging of inflammatory rim, LTPand pathology features were observed. Results Enhancement of inflammatory rim was at peak on 3th, 7th day and reducedon 14th day after RFA. The LTP was performed as crescent and nodular enhancement at arterial phase. Only in the groupD, there was a statistically significant difference of CT value between inflammatory rim in D group and LTP at delayed pha-ses （P〈0.05）. Conclusion The enhanced CT manifestations of rabbit liver VX2 tumor after RFA are consistent withpathologic findings, which can accurately reflect the characteristics of the blood supply and inflammatory changes of RFAand LTP. The findings of inflammatory rim are partly similar with LTP in early stage of post-RFA, so the follow-up CTshould be performed after the inflammation reaction stage.
