320-slice 4D-CTA in assessment of intracranial dural arteriovenous fistulas

摘  要:目的探讨320排4D-CTA诊断及评估颅内硬脑膜动静脉瘘(DAVF)的价值。方法回顾性分析23例经DSA确诊的DAVF患者的4D-CTA资料。由2名医师采用盲法分析4D-CTA图像,评估DAVF部位、供血动脉及静脉引流类型,分析医师间以及4D-CTA与DSA间评估上述指标的一致性。结果 23例患者共27个DAVF,对于判断DAVF部位和引流类型,医师间一致率均为96.30%(26/27),一致性均好(Kappa=0.94),4D-CTA与DSA间一致率均为100%(27/27),一致性均好(Kappa=1.00);医师间及4D-CTA与DSA间检出供血动脉数量差异均无统计学意义(χ2=24.50、21.00,P均>0.05)。结论 320排4D-CTA可准确诊断及评估DAVF部位、供血动脉及静脉引流类型,具有重要临床应用价值。Objective To explore the value of 320-slice 4D-CTA in diagnosis and assessment of intraeranial dural arterio-venous fistulas (DAVF). Methods 4D-CTA data of 23 patients with DAVFs confirmed by DSA were retrospectively re-viewed. The location, the main arterial feeders and the venous drainage type of the DAVFs were independently assessed bytwo neuroradiologists blindly. The inter-observer and inter-modality consistency in assessment of above indicators was ana-lyzed, respectively. Results There were 27 DAVFs in the 23 patients. For assessment of the location and the venousdrainage type of the DAVFs, the consistencies of inter-observer were good (both Kappa = 0. 94) with consistent rate of96.30% (26/27), and the consistencies between 4D-CTA and DSA were also good (both Kappa= 1.00) with consistentrate of 100% (27/27). There were no significantly differences of the main arterial feeder number between different observ-ers as well as 4D-CTA and DSA (X2 = 24.50, 21.00, both P〈0.05). Conclusion 320-slice 4D-CTA is accurate in evaluation of the location, and the main arterial feeders and the venous drainage of DAVFs have important clinical application value.
