鼓室注入庆大霉素明胶海绵浆治疗顽固性梅尼埃病的临床分析

摘　　要：目的探讨不同阶梯浓度的庆大霉素明胶海绵浆治疗顽固性梅尼埃病的临床意义。方法对7例诊断为顽固性梅尼埃病患者并经过常规鼓室注射庆大霉素治疗无效后,通过鼓室内多次注入不同阶梯浓度的庆大霉素明胶海绵浆进行化学性迷路切除,观察治疗后眩晕发作、耳鸣情况、听力变化及并发症。结果 7例中,眩晕3例得到完全控制,3例得到明显缓解,1例无效;耳鸣1例消失,5例缓解,1例无效;听力1例提高,1例加重,其余5例无明显变化。随访1年未发现有鼓膜穿孔。结论该方法可能是治疗顽固性梅尼埃病的一种疗效稳定,安全可靠的新方法。Objective To dicuss the clinical value in the treatment of modified chemical laby-rinthectomy in intractable Meniere＇s disease by repeatedly transtympanic injecting low concentrations of gentamicin gelatin sponge mud . Methods Seven subjects with intractable Meniere＇s disease were includ-ed in this study . Transtympanic low concentrations of gentamicin gelatin sponge mud injecting was taken repeatedly after Ineffective conventional intratympanic gentamyci therapy . he onset of vertigo ,tinni-tus ,hearing change ,and complications were observed after treatment . Results In 7 cases ,complete re-lief vertigo was maintained in 3 cases ,reduced in 3 cases ,unchanged in 1 cases ,and complete relief tinitus was maintained in 1cases ,reduced in 5 cases ,unchanged in 1 cases ,No complications ,such as tympanic membrane perforation or infection were observed . Conclusion Transtympanic repeatedly injecting low concentrations of gentamicin gelatin sponge mud may be a new safe ,reliable modified treatment of intracta-ble M eniere＇s disease .
