摘 要:目的探讨应用超声辐照微泡增强雷帕霉素(RPM)抑制兔腹主动脉球囊损伤后内膜增生的作用。方法选取40只实验兔建立腹主动脉球囊损伤模型,将其随机分为5组,每组8只,分别为对照组、超声+微泡组、单纯RPM组、超声+RPM组及超声+微泡+RPM组。于术前及术后28 d分别超声检查检测腹主动脉内-中膜厚度和全自动生化仪检测血清总胆固醇(TC)、甘油三酯(TG)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、肌酐(CREA)及C-反应蛋白(CRP)水平。术后28 d病理检测内膜与中膜厚度比值。结果术后28 d,超声发现超声+微泡+RPM组内-中膜厚度较其余组减低(P<0.01)。各组间术前、后TG、TC、ALT、AST、CREA水平比较差异均无统计学意义;超声+微泡+RPM组CRP较其余组减低,且术前、后比较差异有统计学意义(P<0.05)。病理检测显示超声+微泡+RPM组内膜与中膜厚度比值减低,与其余组比较差异均有统计学意义(均P<0.01)。结论在兔腹主动脉球囊损伤术模型中,超声辐照微泡介导RPM可抑制内膜增生,降低CRP水平,有望作为临床防治内膜增生的一种新的治疗方法。Objective To evaluate the effect of rapamycin mediated by ultrasound-targeted microbubble destruction on intimal hyperplasia in abdominal aorta after balloon injury in rabbits. Methods Forty male New Zealand rabbits were divided randomly into five groups after the models of injured abdominal aorta with balloon were prepared,including control group,ultrasound +microbubble group,rapamycin group,ultrasound +rapamycin group and ultrasound +microbubble +rapamycin group.Before and 28 day after the operation,ultrasonography was performed to detect the intimal-medial thickness(MT)and blood samples were obtained. Clinical laboratory assessment included total cholesterol(TC),triglyceride(TG),alanine aminotransferase(ALT),aspartate aminotransferase(AST),creatinine(CREA),C-reactive protein(CRP). 28 day after operation,the intima thickness/media thickness(I/M)of aortas were detected using pathological means. Results 28 day after operation,the IMT was thinner in ultrasound+microbubble+ rapamycin group than in other groups(P0.01). There were no significant differences in TC,TG,ALT,AST and CREA before and after operation among all groups. CRP was lower in ultrasound +microbubble +rapamycin group than in other groups and there was significant difference before and after operation(P0.05). Pathological results displayed that I/M was lower in ultrasound+microbubble+ rapamycin group than in other groups(P0.01). Conclusion Rapamycin mediated by ultrasound-targeted microbubble destruction can inhibit the intimal hyperplasia and decrease the CRP level after balloon injury of the abdominal aorta in rabbits,which might provide a new therapeutic method for inhibiting intimal hyperplasia.
