摘 要:目的通过在药学人员中推广品管圈的活动,提高药学专业人员的管理意识及处理和解决问题的能力,降低住院药房药品调配差错次数,保证用药安全。方法 2013年7~12月,以"降低药品调配差错次数"为主题,在我院住院药房按照品管圈活动的10个步骤实施主题选定、活动计划拟定、现状把握、目标设定、解析、对策拟定、对策实施与检讨、效果确认与处置;并评价该活动的实施效果。结果针对药房药品调配差错发生的真因探讨,制定一系列对策包括整理药房看似听似、一品多规药品的品种,制作特殊标识;制作特殊标识标注;改善核对药品的环境,订制药品核对专用桌;制作调配药品与核对药品的指示牌,避免有遗漏药品核对的环节;每周实施差错分享制度;每天对高频易错药品,实施交接班盘点;合理分配摆药时间段,拒绝被催促,排班设计合理化,进行调配准确度考核,与绩效挂钩等。有形成果方面,每月差错次数由之前的44.3件/月减少到9.5件/月;无形成果方面,圈员在愉悦感、解决问题能力、自信心、品管手法等方面有了较大的提高。结论品管圈在降低药品调配差错次数方面是可行有效的,值得推广。Objective To improve the management awareness of pharmacy professionals and the ability of disposing and resolving issues, reduce the number of hotpital pharmacy drug allocation errors, ensure drug safety,through the promotion of QCC activities in the pharmacy staff. Methods It was themed by reducing the number of drag allocation errors in our pharmacy from July to December in 2013. Implemented ten steps of quality control circles :theme selected,program of activities developed and situation grasped, goal set, analysis, countermeasures developed, countermeasures implementation and review, results con- firmed and managed. Evaluated the effectiveness of the activities. Results Developed a series of countermeasures,including arranging pharmacy drugs as seemingly sounding and one species, multi-standard drugs, making special identification mark; im- proving the environment of drug check ,ordering special tables for drugs check, making the signs of drug allocation and recon- ciliation to avoid omitting step of checking drug, implementing error sharing system every week, implementing shift inventory for error-prone drugs of high frequency every day, allocating the time of dispensing drug reasonably,refusing to be urged,ratio- nalizing scheduling design,assossing the accuracy of the allocation that linked to performance and so on. On the hand of tangible outcomes,the number of errors was reduced from 44.3 times / month to 9.5 times / month. On the hand of intangible outcomes, the pleasure, problem-solving skills, self-confidence, quality control techniques and other aspects of members have been greatly improved. Conclusion QCC is a feasible and effective method to reduce the number of drug allocation errors.
