针具打孔单株毛发移植对瘢痕性眉毛缺失的再造

摘　　要：目的 探索烧伤后由于瘢痕导致眉毛缺失的修复再造方法.方法 理发后（保留0.2～0.3 cm长度发长）,在局部麻醉下,顺头发生长方向切取枕后发际区域处含有完整毛发毛囊的皮条,达帽状腱膜浅层,制备毛坯后,依次分割,制备成单株移植物,装入植毛针中备用.以利多卡因麻醉预先设计的眉缺失区域后,按术前设计用医用注射针具制备深约0.3～0.4 cm孔洞.然后通过植毛针将单株毛发按眉的自然走向,植入到眉区预制的孔洞之中.术后至少随访12个月并进行满意度调查.结果 自2011年1月至2012年6月,共完成了15例22侧烧伤后瘢痕性眉毛缺损的再造.所有患者手术均取得满意修复效果.经术后1年以上的临床随访发现：再造后的眉毛生长好,平均成活率达95％.再造的眉外形和走向较自然,接近正常眉毛.患者对术后再造眉形态评价：非常满意者占81.8％;满意者占18.2％.结论 针具打孔单株毛发移植技术手术方法简便快捷,出血少,手术野较清洁,毛囊脱出少,再造的眉毛自然优美,是目前眉毛美容性再造较为理想的方法.Objective To investigate a needle technique for single-hair grafting to repair the eyebrow loss of burn patients.Methods Under local anesthesia,a scalp strip was harvested from the back of the head,close to the hairline.A series of single-hair grafts were then divided under magnification from this strip.Usually,140-180 grafts were needed per eyebrow in a man whereas 100-120 grafts in a woman.When the recipient holes were made with an injective needle in the designed eyebrow area,the single-hair grafts were implanted into the holes one by one,parallel to the original hair directions.The patients were followed up for 12 months and surveyed for their satisfaction.Results Fifteen patients with 22 eyebrows were treated with the above-mentioned technique in only one session operation from January 2011 to June 2012.All of the patients felt satisfied with the results.One-year follow-up revealed that the grafted hairs grew out close to the direction of the original eyebrow,with the average hair survival rate of 95％.The appearance of the reconstructed eyebrows was quite natural,close to the normal eyebrow.As for the appearance of their reconstructed eyebrows,81.8％ of the patients felt very satisfied and 18.2％ felt satisfied.Conclusion The above-mentioned technique is a simple,fast and effective method with less bleeding for eyebrow reconstruction in burn patients.It may be an ideal method for eyebrow reconstruction in burn patients.
