摘 要:目的 观察卵巢切除对硫酸鱼精蛋白介导的间质性膀胱炎(IC)大鼠模型中膀胱肥大细胞的影响,探讨雌激素、孕激素在IC发病中的作用.方法 将30只雌性成年SD大鼠随机分为3组:卵巢切除+硫酸鱼精蛋白(OVX+ PS)组、假手术+PS(Sham+ PS)组和正常对照组.OVX+ PS组和Sham+ PS组膀胱内灌注PS建立IC膀胱黏膜损伤机制大鼠模型,正常对照组膀胱内灌注生理盐水.60 d后处死大鼠并切取膀胱,苏木素-伊红(HE)染色观察膀胱黏膜变化及炎性细胞浸润,甲苯胺蓝染色标记并计数肥大细胞,分析肥大细胞活性.结果 PS灌注组可见膀胱黏膜损伤,黏膜下可见炎性细胞浸润.OVX+ PS组膀胱黏膜固有层和逼尿肌肥大细胞计数分别为(2.770±1.112)、(1.180±0.598)个/高倍镜视野,Sham+ PS组肥大细胞计数分别为(2.640±1.022)、(0.990±0.630)个/高倍镜视野,与正常对照组[(1.160±0.523)、(0.330±0.177)个/高倍镜视野]比较均明显增多(P<0.01);OVX+ PS组与Sham+ PS组比较肥大细胞计数差异无统计学意义(P>0.05).OVX+ PS组膀胱黏膜固有层和逼尿肌脱颗粒肥大细胞百分比分别为(27.46±15.69)%和(36.93±19.64)%,与Sham+ PS组[(90.94±8.46)%、(80.58±24.18)%]比较明显降低(P<0.01);与正常对照组[(22.92±9.52)%、(24.95±18.81)%]比较,OVX+ PS组膀胱黏膜固有层和逼尿肌的脱颗粒肥大细胞百分比差异均无统计学意义(P>0.05),而Sham+ PS组脱颗粒肥大细胞百分比均明显增加(P<0.01).结论 PS大鼠膀胱灌注可构建较为典型的IC动物模型;卵巢切除可显著抑制IC模型中膀胱肥大细胞的活化及脱颗粒.Objective To investigate the role of female hormones in the pathogenesis of interstitial cystitis by the study of ovariectomy on the bladder mast cells in animal models of interstitial cystitis.Methods A total of 30 female adult Sprague-Dawley rats were divided into 3 groups,including bilateral ovariectomy plus protamine sulfate(OVX + PS) group,sham operation plus protamine sulfate control (Sham +PS) group and control group.The OVX + PS group and Sham + PS group were instilled with 50 g/L protamine sulfate once every two days,and the control group was instilled with normal saline.After intravesical instillation for 60 days,the rats were killed and the bladders were removed for toluidine blue staining to count the number of mast cell and to investigate the form of mast cells.Results In the groups of PS instilled,the bladder mucosa was damaged,and inflammatory cells could be seen in lamina propria.In OVX + PS group,the number of mast cells in lamina propria and detrusor was (2.770 ± 1.112)/HP and (1.180 ±0.598)/HP respectively,and the number was (2.640 ± 1.022)/HP and (0.990 ±0.630)/HP in Sham + PS group.Both of the number were obviously higher than those in control group [(1.160 ± 0.523)/HP and (0.330 ± 0.177)/HP,respectively],and the difference was statistically significant(P 〈 0.01).However,compared to OVX + PS group and Sham + PS group,the difference was not statistically significant (P 〉 0.05).the OVX + PS group showed significant lower of the percentage of degranulated mast cells [(27.46 ± 15.69) % and (36.93 ± 19.64)% in lamina propria and detrusor respectively] than those in Sham + PS group [(90.94 ± 8.46)% and (80.58 ± 24.18)%],and the difference was statistically significant (P〈0.01).Compared to control group [(22.92 ± 9.52) % and (24.95 ± 18.81) %],the percentage of degranulated mast cells in Sham + PS group was obviously higher,and the difference was statistically significant (P 〈0.01).Compa
