江苏省高邮市门诊患者卫生服务需求与利用影响因素调查分析

摘　　要：目的了解江苏省高邮市医疗卫生单位门诊患者卫生服务需求与利用的相关影响因素，为农村卫生适宜技术推广及医疗保障制度的施行提供科学信息参考。方法采用统一问卷，从高邮市医疗卫生单位抽取5家医院进行门诊患者一日现况调查。结果有效问卷的相关分析显示：44．3％的门诊患者就诊费用在100元以下，100～200元的占到30．1％，300元以上的占到15．4％，200—300元的人数最少（10．2％）。就诊费用与调查对象的性别、就业状况、就诊医院级别、科室、疾病类别及药费相关。55．1％的门诊患者认为就诊流程简易，41．7％的认为一般，认为复杂的有3．1％；对医生的技术水平评价中，好评的占89．5％，其他10．5％的人认为一般。患者对医生技术水平评价与就诊医院级别、能否报销、疾病类别及年龄相关。结论应针对不同人群的特点，在不同级别的医院，综合各种影响因素，广泛开展新型农村合作医疗等医疗保障制度，合理选择卫生适宜技术，提高基层医院医疗服务质量，降低医疗费用，真正解决农民看病难、看病贵的问题。Objective To survey the influence factors related to the health services demand and utilization in out-patients from medical and health unit in Gaoyou City, and provide scientific information for the promotion of appropriate technology and implementation of medical insurance system in rural areas. Methods A questionnaire survey was performed in the outpatients from five hospitals in Gaoyou City. Results The medical costs was less than 100 yuan in 44.3% of patients, 100 -200 yuan in 30.1% of them,200 -300 yuan in 10.2% of them,over 300 yuan 15.4% of them. The gender,em- ployment status, hospital level,department, disease classification and drug costs were the related factors for the medical costs. 55.1% of outpatient thought the medical process was simple,41.7% generally, and 3.1% complex; 89.5% of them agreed that the technical level of the doctors was favorable, 10.5 % general. The evaluation on the technical level of doctors was related to hospital grade,medical insurance,types of disease and age. Conclusion We should aim at the dif- ferent characteristics of patients, take comprehensive consideration of influence factors, extensively carry out New Rural Cooperative Medical Care and other medical security system at different levels of the hospital, adopt the appropriate health technology for rural area,improve the quality of primary medical care, reduce medical costs, and really solve the problem of farmers visiting doctor being difficult and expensive.
