外伤后勃起功能障碍行三件套膨胀性假体植入术一例报告并文献复习

摘　　要：目的 探讨三件套膨胀性阴茎假体植入术治疗外伤后勃起功能障碍的安全性和有效性.方法 外伤后阴茎勃起困难伴尿道狭窄1年余患者1例,术前进行严格的皮肤准备及预防性使用抗生素.以阴茎阴囊交界处为横行切口,扩张好海绵体腔后,测量好两个海绵体腔各为16cm,选择美国AMS 700 CXR三件套膨胀性阴茎假体,圆柱体长度为14cm,两侧圆柱体分别加2cm延长帽1个,分别置入两侧的海绵体腔内,用抗生素盐水冲洗,于膀胱左侧潜行分离出一个腔隙,置入贮水囊,水囊内注入生理盐水60mL,外力增加腹压时检查贮水囊无返流.将圆柱体和水囊与泵用管道相连通,将泵置于阴囊肉膜下.结果 术后第1d血压、血糖高,经积极处理后,降至正常;术后7d术口及阴囊出现坠痛,对症处理1个月后缓解;术后14d拔除尿管,尿线稍细,1个月后每周行尿道扩张一次,共4次,现排尿正常;术后20d出院,8周后开始尝试性交,随访结果夫妻双方均满意.结论 AMS700CXR三件套可膨胀性阴茎假体更接近人体生理,隐蔽性好,使用方便,手术创伤小,安全性高,患者满意.Objectives To summarize the safety and availability of three-piece dilatability penile prosthesis of implantation was treated erectile dysfunction after trauma.Methods The patient that erectile dysfunction with urethral stricture for one year after trauma,strict skin preparation and prophylactic use of antibiotics in preoperative.In penis scrotal junction for transverse incision,expanded sponge body cavity,measured two sponge body cavity was 16 cm,selected AMS 700 CXR,cylinder length is 14 cm,on both sides of the cylinder to add 2 cm respectively extend cap 1,were inserted on both sides of the cavernous cavity,rinsed with antibiotics brine.To isolate a lacuna on the left side of the bladder,placed water storage pouch,injected physiological saline with 60 ml in water pouch,when external force increase abdominal pressure check water storage capsule without reflux.The cylinder and water sac connect pipe and pump,under the scrotal dartos.Results It has high blood pressure and blood sugar on the first day,and dropped to normal after active treatment; It appeared falling pain with scrotum in 7 days,eased with symptomatic treatment in 1 month; Pulled out ureter in 14 days,and urinary line is thinner,It has urethra expansion once a week for a month,four times a total,now,micturate is normally ; Hospital discharged after 20 days,and,trying to sexual intercourse after 8 weeks,satisfied with both sides of husband and wife follow-up results.Conclusions AMS 700 CXR is more close to physiological with human,good concealment,easy to use,surgical trauma is smaller,high safety,and Satisfied with the patient.
