五志护理对慢性心力衰竭并抑郁患者生存质量的影响

摘　　要：目的了解慢性心力衰竭并抑郁患者生存质量状态，探讨五志护理的干预效果。方法选取2009年12月～2013年10月入住广西中医药大学第一附属医院心内科治疗的慢性心力衰竭患者，人院后行Zung抑郁自评量表（SDS）、汉密尔顿抑郁量表（24项，HAMD）、生存质量测定量表简表（WHO，QOL—BREF）评估抑郁状态和生存质量，根据评测结果，将86例慢性心力衰竭并抑郁患者分为对照组42例和五志护理组44例。对照组行内科常规护理，五志护理组在对照组基础上强化五志护理，3周后观察并比较两组患者抑郁状态和生活质量的改善效果。结果五志护理组与对照组抑郁情绪得分比较，差异有统计学意义（P＜0．05），五志护理组患者的QOL—BREF量表得分除环境领域外，均明显高于对照组患者，差异有统计学意义（P＜0．05）。结论五志护理可以明显改善慢性心力衰竭患者的抑郁状态，并提高患者生存的质量。Objective To investigate the quality of life in patients with chronic heart failure and depression state, to in- vestigate the effect of five minds nursing. Methods Chronic heart failure patients from December 2009 to October 2013 in Department of Cardiology in the First Affiliated Hospital of Guangxi Traditional Chinese Medicine University admitted for treatment. Zung Self-rating Depression Scale （SDS）, Hamilton Depression Scale （24 items, HAMD）, WHO QOL-BREF assessment of depression and quality of life were used. According to the evaluation results, the chronic heart failure and depression of 86 patients were divided into the control group of 42 cases and five minds nursing of 44 patients. The control group underwent routine nursing care, five minds nursing group enhanced five minds nursing based on the control group. After 3 weeks of nursing, the quality of llfe and depression improving effect were observed and compared between the two groups. Results Depression scores of the two groups had statistically significant difference （P〈0.05）, QOL-BREF scores in five minds nursing group were significantly higher than those in control group except the environment （P〈0.05）. Conclusion Five minds nursing can significantly improve the depressive state in patients with chronic heart failure, and improve the life quality of patients.
