正压通气治疗老年急性心力衰竭的临床效果

摘　　要：目的 探讨正压通气治疗老年急性心力衰竭的临床效果。方法 回顾性分析2013年1~12月于本院实行抢救的86例老年急性心力衰竭患者,其中43例实行常规抗急性心力衰竭治疗的患者归为对照组,43例在常规治疗的基础上再给予正压通气治疗的归为研究组,比较两组患者的心功能、肺功能各项指标和存活率。结果 与对照组相比,研究组患者LVEF、LVEDD及LVESD等心脏功能指标显著改善,差异有统计学意义（P〈0.01）;研究组患者血氧饱和度、动脉血氧分压、指脉氧饱和度及呼吸频率等肺功能指标显著改善,差异有统计学意义（P〈0.01）。研究组患者的存活率显著高于对照组（P〈0.05）。结论 正压通气可有效改善老年急性心力衰竭患者的心肺功能,提高存活率。Objective To explore the clinical effect of positive airway pressure on rescuing elderly patients with acute heart failure. Methods Retrospective analysis was applied to 86 elderly patients with acute heart failure who were rescued in our hospital from January to December 2013.Patients treated by regular acute heart failure resistance were clas- sified into control group （n=43）,while the other patients treated by positive airway pressure on the basis of regular treatment were categorized into research group （n=43）.Indexes of the cardio-pulmonary function,and survival rate were compared in both groups. Results Compared with control group,indexes of cardiac function including left ventricular e- jection fraction （LVEF）,left ventricular end-diastolic dimension （LVEDD）,and left ventricular end systolic diameter （LVESD） were obviously improved in the research group,which displayed statistical differences in the two groups （P〈 0.01）.In research group,indexes of pulmonary function including oxyhemoglobin saturation,arterial partial pressure of oxygen,blood oxygen saturation of artery blood of finger and respiratory rate were greatly improved,which also displayed statistical differences in the two groups （P〈0.01）.The survival rate in the research group was much higher than that in the control group （P〈0.05）. Conclusion Positive airway pressure can effectively improve cardio-pulmonary function in elderly patients with acute heart failure and increase the survival rate.
