隔姜隔盐灸神阙穴治疗缺血性中风后急迫性尿失禁45例随机对照临床观察

摘　　要：目的观察隔姜隔盐灸神阙穴治疗缺血性中风后急迫性尿失禁的临床疗效。方法将72例缺血性中风后急迫性尿失禁住院患者按照2∶1的比例随机分为治疗组48例和对照组24例。对照组给予针刺治疗及西医基础治疗,针刺取双侧合谷、内关、曲池、三阴交、足三里、阳陵泉,每次留针30min,每周5次;西药给予拜阿司匹林0.1g,每日1次;波立维75mg,每日1次,共治疗4周。治疗组在对照组治疗基础上加用隔姜隔盐灸神阙穴,每周5次,共治疗4周。观察两组患者治疗前后平均白天排尿次数、夜间护理人员被叫次数,日常生活能力评定量表(Barthel指数评分)及尿失禁程度量表评分。结果与本组治疗前比较,治疗组治疗后平均白天排尿次数及夜间护理人员被叫醒次数减少,尿失禁程度量表评分均降低,Barthel指数评分提高(P<0.01);两组治疗后比较,治疗组平均白天排尿次数、夜间护理人员被叫醒次数减少及尿失禁程度量表评分均较对照组明显降低(P<0.01),而Barthel指数评分比较差异无统计学意义(P>0.05)。结论隔姜隔盐灸神阙穴可以有效改善缺血性中风后患者的尿失禁,提高患者生活质量。Objective To observe the therapeutic effect of ginger-salt-separated moxibustion on Shenque（ CV 8）for urge incontinence after ischemic stroke. Methods Totally 72 cases of ischemic stroke were randomized into the treatment group of 48 cases and control group of 24 cases. The control group was given acupuncture on bilateral Hegu（ LI4）,Neiguan（ PC6）,Quchi（ LI11）,Sanyinjiao（ SP6）,Zusanli（ ST36） and Yanglingquan（ GB34） and western medicine including 0. 1 g Aspirin and 75 mg Plavix once daily for 4 weeks. The treatment group was added with ginger-salt-separated moxibustion on Shenque（ CV 8） five times weekly for 4 weeks. The frequencies of daytime and night urination and the scores of Barthel Index and incontinence degree were observed in both groups before and after treatment. Results The frequencies of daytime and night urination and the score of incontinence degree were significantly decreased but the score of Barthel Index was significantly increased in the treatment group after treatment（ P〈0.01）. There were significant differences between groups in the frequencies of daytime and night urination and the score of incontinence degree after treatment（ P〈0. 01）. There was no significant difference between groups in the score of Barthel Index after treatment（ P〈0. 05）. Conclusion Ginger-salt-separated moxibustion on Shenque（ CV 8） can effectively relieve urge incontinence and improve life quality of ischemic stroke patients.
